吞咽障碍患者家属参与康复护理方案的研究

摘　　要：目的探讨科学、实用的患者家属参与吞咽障碍患者康复护理方案。方法基于文献检索和吞咽障碍患者康复训练调查分析,结合现有的吞咽障碍患者康复方案,制订吞咽障碍患者家属参与康复护理方案的草案;选取18名10年以上工作经验的护理专家,进行2轮德尔菲专家咨询,修订康复护理方案草案。结果 2轮专家咨询问卷的回收率均为100%,权威程度为0.84,协调系数为0.52,协调系数的显著性检验结果为P<0.05,评价结果可取。制订的吞咽障碍患者家庭康复护理方案,包括3项一级指标,8项二级指标,25项三级指标,内容涉及患者家属参与内容,医务人员协助参与的职责,以及保障实施的建议。结论患者家属参与吞咽障碍患者康复护理方案的制订方法科学、简单易懂,可为神经内、外科吞咽障碍的患者提供临床技术指导。Objective To explore a scientific and practical nursing plan for family members of patients with swallowing disorders to involve in their rehabilitation. Methods Based on literature search as well as investigation and analysis on the rehabilitation training of patients with swallowing disorders,combined with the existing rehabilitation nursing plan of patients with swallowing disorders,a draft for the family-participating rehabilitation nursing plan of patients with swallowing disorders was developed. Eighteen nursing specialists with more than 10 years work experience were selected,and two rounds of Delphi expert consultation were taken,and the draft of the rehabilitation nursing plan was revised. Results The questionnaire collecting rates of the two rounds of expert consultation were both 100% ,the authority degree coefficient was 0.84 and the coordination coefficient was 0.52. The significant test result of the coordination coefficient was P〈0.05. The evaluation results were desirable. The family-participating rehabilitation nursing plan of patients with swallowing disorders developed by this paper contained 3 first-grade indicators,8 second-grade indicators and 25 third-grade indicators,covering the following aspects such as the participating contents for patients＇ family members,the assisting duties of medical workers and the suggestions on implementation guarantee. Conclusion The way to establish the family-participating rehabilitation nursing plan of patients with swallowing disorders is scientific and easy to understand. It also provides a clinical technical guidance for neurological and neurosurgical patients with swallowing disorders.
