芪苈强心胶囊治疗舒张性心力衰竭的疗效及安全性的Meta分析

摘　　要：目的:系统评价芪苈强心胶囊治疗舒张性心力衰竭的疗效及安全性。方法:计算机检索PubMed、EMbase、The Cochrane Library(2015年第3期)、CBM、CNKI、VIP和万方数据库,收集芪苈强心胶囊治疗舒张性心衰的随机对照试验(RCT)。由2位评价员分别筛选文献、提取资料和方法学质量评价后,采用RevMan 5.2软件进行Meta分析。结果:最终纳入18个RCT,共计1 404例患者。Meta分析结果显示:与对照组治疗相比,联合使用芪苈强心胶囊可以降低舒张性心衰患者血浆NT-proBNP水平,显著改善心脏舒张功能,提高临床治疗有效率和中医症候有效率,且无明显不良反应。结论:芪苈强心胶囊能够降低舒张性心衰患者血浆NT-proBNP水平,改善舒张功能,提高舒张性心衰治疗的有效率。Objective：To evaluate the efficacy and safety of Qiliqiangxincapsule,a traditional Chinese medicine,in diastolic heart failure（DHF）therapy.Methods：Randomised controlled trials（RCTs）of DHF treated with Qiliqiangxin capsule were searched from PubMed,EMbase,the Cochrane Library（Issue 3,2015）,CBM,CNKI,VIP and Wanfang Data up to March,2015.The studies included were those screened by two researchers independently according to the exclusion and inclusion criteria and meta-analysis was conducted by using RevMan 5.2software after data extraction and quality assessment.Results：Eighteen RCTs were included involving 1 404 patients.The results of meta-analysis showed that compared with normal treatment,combination of Qiliqiangxincapsule treatment could decrease serum NT-proBNP level（MD=-266.47,95%CI [-347.29,-185.64]）,P0.01;improve cardiac diastolic function,E/A（MD=0.16,95%CI [0.10,0.23]）,E/E＇（MD=-1.97,95%CI[-2.65,-1.30]）,P〈0.01;increase the clinical efficacy（RR=1.29,95%CI [1.21,1.36]）and the traditional Chinese medical（TCM）symptoms efficacy（RR=1.24,95%CI [1.14,1.35]）,P〈0.01.No significant side effect of Qiliqiangxincapsule had been reported during DHF treatment.Conclusion：Current evidence shows that Qiliqiangxincapsule treatment can reduce serum NT-proBNP level,amend cardiac diastolic function,and improve both clinical efficacy and TCM symptom,but the long-term prognosis,especially the mortality and rehospitalization rate,needs to be further studied and evaluated.Due to the limited quantity and quality of the included studies,the conclusion above needs to be further confirmed by more high-quality,multi-centre RCTs with large sample size.
