消脂护肝胶囊对非酒精性脂肪性肝炎患者脂联素和胰岛素抵抗的影响

摘　　要：目的:观察消脂护肝胶囊治疗非酒精性脂肪性肝炎(NASH)的临床疗效。方法:收取60例符合入选标准的患者为研究对象,随机分为治疗组和对照组,各30例。所有患者均在运动、饮食指导等基础上给予药物治疗。治疗组患者口服消脂护肝胶囊(院内制剂)。对照组患者口服多烯磷脂酰胆碱(易善复)胶囊。疗程均为3个月,并随访3个月。检测并观察各组患者治疗前后肝功能(ALT、AST)、血脂(TC、TG)及脂联素(ADPN)、胰岛素抵抗指数(IRI)等细胞因子水平变化情况。结果:1两组患者血清ADPN水平治疗后较治疗前均显著升高(0.05),且治疗组与对照组比较升高更为明显,差异有显著性意义(0.05)。2治疗组患者血清IRI水平治疗后较治疗前明显降低(0.05)。3两组患者治疗后肝功能(ALT、AST)及血脂(TC、TG)水平与治疗前比较均明显降低(<0.05),且治疗组患者ALT、TG水平与对照组比较降低更为显著,差异有显著性意义(0.05)。结论:消脂护肝胶囊联合饮食运动疗法治疗NASH,在提高患者血清ADPN、降低ALT、AST、TG、TC和IRI等疗效指标方面效果显著,值得临床推广。Objective：To observe clinical efficacy of Xiaozhi Hugan capsules on non-alcoholic steatohepatits（NASH）. Methods： Sixty patients diagnosed with NASH enrolled the study, and were randomly divided into the test group（n=30） and control group（n=30）. All patients received drug treatment based on exercise and managed diet. The test group was treated with Xiaozhi Hugan capsules（hospital preparations）, the control group was treated with polyene phosphatidylcholine（Essentiale） capsules. Course of treatment was three months, and follow-up period was three months. Several inspections were detected and observed in each group before and after treatment, such as liver function（ALT and AST）,blood fat（TC and TG）, serum adiponectin（ADPN） and insulin resistance index（IRI）. Results： 1After treatment, ADPN increased significantly（P〈0.05）, ADPN increase of the test group was more significant（P〈0.05）. 2After treatment, IRI of the test group was decreased（P〉0.05）. 3ALT, AST, TC and TG in the test group significantly decreased after treatment（P〉0.05）, ALT and TG decrease of the test group were more significant（P〈0.05）. Conclusion： Xiaozhi Hugan capsule combined with exercise and managed diet has significant therapeutic effect on NASH, such as the improvement of serum ADPN and the reduction of ALT, AST, TG, TC and IRI, and worthwhile for spreading in clinical practice.
