人工全髋关节置换术治疗快速破坏性髋关节病疗效分析

摘　　要：[目的]探讨人工全髋关节置换术治疗快速破坏性髋关节病的临床治疗效果。[方法]回顾性分析12例临床非常少见的快速破坏性髋关节病的临床资料,分析手术特点,评价手术治疗效果。[结果]本组12例患者,占同期行髋关节置换手术患者的0.3%。其中男11例,女1例;平均年龄(49±12)岁。从最初出现髋关节疼痛的症状到影像学检查股骨头消失时间平均为(9±3)个月。全部使用生物固定型假体,术后平均随访(37±12)个月,无松动失败病例。Harris评分从术前平均(20±2)分提高到术后(97±3)分(P<0.01)。临床治疗效果满意。[结论]快速破坏性髋关节病临床上非常少见,病情进展迅速,破坏严重。人工全髋关节置换术是治疗的有效方法。[Objective] To analysis the efficiency of total hip arthroplasty（ THA） in treatment of rapidly destructive arthropathy of the hip joint（ RDA）. [Methods] Twelve patients with RDA（ 11 male,1 female,average 49 ± 12 years old）were treated by THA. The period from the onset of the symptom to the complete osteolysis of femoral head was averagely（ 9 ± 3）months. All cases received biological cementless prosthesis fixation. Harris score was applied before and after operation to evaluate the clinical effect. SPSS16. 0 sofeware was used for statistic analyses. [Results] This series of 12 patients accounted for about0. 3% of all the THA patients in our hospital at the same period. The average follow up time was（ 37 ± 12） months,all cases were in good function and no failure case was found. The Harris score improved from（ 20 ± 2） preoperatively to（ 97 ± 3） postoperatively（ P 〈0. 01）. [Conclusion] The RDA is very rarely seen in clinics. THA is an effective method to treat RDA.
