摘  要:目的探讨慢性阻塞性肺疾病(COPD)患者白细胞介素-32(IL-32)、白细胞介素-17(IL-17)、和肽素(COPEPTIN)水平变化与吸烟对肺功能损伤的相关性。方法按照一定的纳入排除标准将100例COPD患者纳入到本研究中,并按照是否吸烟分为COPD吸烟组和COPD不吸烟组,同时将100名不吸烟的肺功能正常的健康体检者纳入到健康对照组中,分别在急性加重期和稳定期检测COPD患者血清和诱导痰中IL-32、IL-17、COPEPTIN水平和肺功能情况,同时检测健康对照组L-32、IL-17、COPEPTIN水平,分析吸烟、肺功能与上述炎症指标的相关性。结果 COPD吸烟组肺功能与COPD不吸烟组相比,差异有统计学意义(P<0.05);急性加重期与稳定期相比,COPD患者血清、诱导痰IL-32、IL-17、COPEPTIN水平均显著升高(P<0.05);COPD吸烟组血清IL-17、COPEPTIN水平显著高于COPD不吸烟组和健康对照组(P<0.05),COPD吸烟组诱导痰中IL-32水平显著高于COPD不吸烟组和健康对照组(P<0.05);COPD不吸烟组血清IL-17、COPEPTIN水平显著高于健康对照组(P<0.05)。Logistic回归分析表明血清IL-17、COPEPTIN与诱导痰IL-32水平是影响COPD患者肺功能的独立危险因素(P<0.05),血清IL-17、COPEPTIN与诱导痰IL-32水平越高,患者肺功能越差。线性回归结果表明IL-17、COPEPTIN、诱导痰IL-32水平分别与吸烟数成正相关(P<0.05),r分别为0.679、0.735、0.793,b分别为0.041、0.050、0.057。结论吸烟可以导致COPD患者血清IL-17、COPEPTIN与诱导痰IL-32表达的增高,即引起肺组织局部和全身的炎症反应,从而造成患者肺功能的进一步降低。Objective To discuss the correlation between level changes of interleukin-32( IL-32),interleukin-17( IL-17),COPEPTIN in patients with chronic obstructive pulmonary disease( COPD) and smoking,lung function injury. Methods According to certain inclusion and exclusion criteria,100 cases of COPD patients were included in the study,and according to smoking or not,they were divided into COPD smoking group and COPD non-smoking group,at the same time,the 100 nonsmokers with normal lung function of healthy people were included into healthy control group; IL-32,IL-17,COPEPTIN levels of serum and induced sputum,and pulmonary function in COPD patients were given detection in the acute exacerbation and stable stage,at the same time,L-32,IL-17,COPEPTIN levels of study objects of health control group were detected,so that the correlation between smoking,pulmonary function and inflammatory markers above were analyzed. Results By comparison of pulmonary function of COPD smoking group with that of COPD nonsmoking group,the difference was statistically significant( P 〈 0. 05); By comparison of the acute exacerbation stage with stable stage,IL-32,IL-17,COPEPTIN levels of serum and induced sputum in COPD patients were significantly elevated( P 〈 0. 05); Serum IL-17,COPEPTIN levels of COPD smoking group were significantly higher than that of COPD non-smoking group and healthy control group( P 〈 0. 05),induced sputum IL-32 levels of COPD smoking group were significantly higher than that of COPD non-smoking group and healthy control group( P 〈 0. 05); serum IL-17,COPEPTIN levels of COPD non-moking group were significantly higher than those of healthy control group( P 〈 0. 05). Logistic regression analysis showed that serum IL-17,COPEPTIN and induced sputum IL-32 levels were independent risk factors influencing the pulmonary function of patients with COPD( P 〈 0. 05),serum IL-17,COPEPTIN and induced sputum IL-32 levels were higher,the pulmonary function of patients was worse.
