摘 要:目的:观察复合辅酶辅助治疗新生儿窒息后心肌损害的临床疗效。方法:选取2011年1月至2013年12月我院收治的窒息后心肌损害新生儿70例,随机分为对照组31例和观察组39例。两组均给予常规治疗,包括保持气道通畅、吸氧、维持酸碱平衡及水电解质平衡、防治惊厥、改善循环、心动过缓者加用多巴酚丁胺、心力衰竭时予利尿剂和强心药等;观察组加用复合辅酶(100单位/支)100单位/次,1次/天,加入8%葡萄糖氯化钠20 m L稀释后静脉泵注。两组均治疗1周为一疗程。患儿于入院24 h内及疗程结束后第2天取静脉血,应用全自动生化分析仪测定肌酸激酶同工酶(CK-MB),酶放大化学发光仪测定肌钙蛋白(Tn-T),检查心电图,观察临床疗效。结果:观察组总有效率89.74%,高于对照组的64.52%(P<0.05);观察组CK-MB与Tn-T的下降幅度大于对照组(P<0.01)。结论:常规治疗基础上加用复合辅酶治疗新生儿窒息后心肌损害,有利于临床症状、心电图、心肌酶谱的恢复,提高疗效。Objective: To observe the clinical efficacy on treatment of myocardial injury after asphyxia in neonates with coenzyme complex. Methods: From Jan. 2011 to Dec. 2013,seventy neonates with myocardial injury after asphyxia admitted in Jiangshan Maternity and Child Care Hospital were randomized into the control group( 31 cases) and observation group( 39 cases). The cases in the control group were treated with conventional therapy,and the cases in the observation group were treated with conventional therapy combined with coenzyme complex therapy. The two groups were given conventional therapy,including maintaining airway patency,uptake oxygen,maintain acid-base balance and water electrolyte balance,control the convulsion,improve the circulation,bradycardia infants given the dobutamine and heart failure infants treated with diuretics and cardiac medicine etc.. The observation group were treated with compound coenzyme( 100 units / Branch),100 units / time,once a day,8% glucose and sodium chloride is added to the diluted 20 m L intravenous infusion. The two groups were treated for 1 week as a course of treatment. Children were collcetde the venous blood within 24 hours and at the second day after the course. The clinical symptom improvement,ECG and variations in CK-MB and Tn-Tin both groups were monitored. Results: The total effective rate for the observation group and the control group was 89. 74% and64. 52 %,respectively,and the difference between the two groups was statistically significant( P〈0. 05). Decreases in CK-MB and Tn-T for the observation group were significantly more compared to the control group,and the differences between the two groups were statistically significant( P〈0. 01). Conclusion: Conventional therapy combined with coenzyme complex therapy is more effective in improving the symptoms and ECG of myocardial injury after asphyxia in neonates as well as in restoring myocardial zymogram.
