二级临床诊疗科目专业内容与DRGs范围的实证分析

摘　　要：目的以呼吸内科专业为例，比较北京版DRGs的MDC所覆盖的疾病诊疗内容与我国现行二级临床诊疗科目开展的专业服务内容。方法通过北京地区二级以上医院2012年至2014年病案首页数据，基于帕累托图统计学方法界定的DRGs，以及专家论证的DRGs，二者结合对呼吸内科专业涉及的DRGs范围进行分析界定。结果通过两种方法结合界定的呼吸内科涉及的DRGs为42个组。结论医院二级临床科室DRGs范围界定应通过专家咨询和临床数据统计学方法共同确定。Objective Taking the respiratory department of internal medicine as an example, to compare the coverage of clinical treatment of the MDC covered by DRGs of Beijing version with the professional services offered as secondary clinical treatment subjects in China. Methods Using the data from medical record home page from hospitals in Beijing above secondary level from 2012 to 2014 and both the DRGs defined in Pareto diagram statistical method and the DRGs proved by experts, for analysis and definition of the DRGs coverage involved by respiratory discipline of internal medicine. Results Respiratory discipline of internal medicine involved DRGs of 42 groups as found by the two methods. Conclusion The DRGs scope of secondary clinical departments in hospitals should be made based on both expert consultation and clinical data statistics method.
