抗C1q抗体与系统性红斑狼疮及狼疮肾炎的相关性

摘　　要：目的研究系统性红斑狼疮(systemic lupus erythematosus,SLE)患者抗C1q抗体与疾病活动及狼疮性肾炎(lupus nephritis,LN)的相关性.方法采用ELISA法测定62例SLE患者和47例其他风湿性疾病患者及21名健康对照者血清中抗C1q抗体的浓度.62例SLE分为疾病活动组(36例)和疾病稳定组(26例),并按LN判定标准分为LN组(32例)和非LN组(30例).分析各组抗C1q抗体水平与SLE患者补体指标、SLE活动性评分间的相关性.结果 SLE患者中血清抗C1q抗体阳性率为64.51%,显著高于疾病对照组和正常对照组(P<0.05).SLE活动组抗C1q抗体水平显著高于稳定组(P<0.01).LN组患者血清抗C1q抗体的水平显著高于非LN组SLE患者(P<0.01).血清中抗C1q抗体水平与SLEDAI评分、抗ds DNA抗体水平显著正相关,与补体C3、C4含量显著负相关.结论 C1q抗体水平能反映SLE的疾病活动程度,并与SLE肾损害相关,对活动性LN敏感度高.Objective To investigate the associations between anti- C1 q antibodies and systemic lupus erythematosus（SLE） and that between anti- C1 a antibodies and lupus nephritis（LN）.Methods The Anti- C1 q antibodies were measured using ELISA kit from 62 SLE patients, 47 patients with other rheumatic disease and 21 healthy cases.The 62 SLE patients were divided into active group with 36 patients and stable groups with 26 patients.The level of anti- C1 q antibodies was detected, then the correlation between the level of anti- C1 q antibodies and the results of laboratory detection for SLE patients as well as the Disease Activity Indexes（SLEDAI）is analyzed.Results The positive rate of anti- C1 q antibodies in SLE patients was 64.51%, which was significantly higher than those in other control groups（P〈0.05）.The levels of anti- C1 q antibodies in patients with active SLE were significantly higher than its counterpart in patients with inactive SLE（P〈0.01）.The serum levels of anti- C1q（P〈0.01） in the LN group was significantly higher than those in the non LN group.Serum levels of anti- C1 q antibodies showed a strongly positive correlation with SLEDAI score, anti- ds DNA antibodies, and showed a negative correlation with C3 and C4.Conclusion Anti- C1 q antibodies can characterize the disease activity of SLE,and are associated with SLE damage,specifically with active LN.
