结构脂肪乳剂对大面积褥疮早期治疗氮平衡的影响

摘　　要：目的:观察结构脂肪乳剂(STG)和中/长链脂肪乳剂(MCT/LCT)对大面积褥疮患者氮平衡的影响。方法:将大面积褥疮患者24例随机分为STG组和MCT/LCT组各12例,于住院后第3日开始接受5 d等氮等热量肠外营养。STG组使用结构脂肪乳剂,MCT/LCT组使用物理混合的中/长链脂肪乳,对比观察两组患者氮平衡情况。结果:两组病人在治疗前均存在明显的负氮平衡,从治疗后第2日起与治疗前对比,治疗后氮平衡明显改善,两组均有统计学差异;两组间比较,从治疗后第3日起,具有显著差异,STG组积氮平衡值明显高于MCT/LCT组。结论:STG和MCT/LCT均对改善氮平衡有益,结构脂肪乳具有更好的节氮作用,能有效地改善大面积褥疮患者的蛋白质代谢,有利于大面积褥疮患者更快地恢复。Objective： To study the influence of structured triglyceride on nitrogen balance in surgical patients with severe bedsores. Methods： A prospective,random clinical trial was performed,and 24 surgical patients with severe bedsores were randomly divided into two groups according to the treatment of structured triglyceride（ STG） or physical mixture of medium- and long-chain triglycerides（ MCT / LCT） emulsion. Both groups received isocaloric parenteral nutrition（ PN） in the five days from the 3rd day of be in hospital. The nitrogen balance was compared between the two groups. Results： The nitrogen balance in each group was improved from the 2nd day of PN. In the same way,the nitrogen balance in the STG group was significantly higher than that in the MCT / LCT group. Conclusion： Although Both STG and MCT / LCT are good for the nitrogen balance,STG can improve the nutrition of surgical patients with severe bedsores more rapidly than MCT / LCT
