摘 要:目的:探讨脓毒血症患者血浆B型钠尿肽(BNP)、高敏C反应蛋白(hs-CRP)的变化及其临床意义。方法:选择2013年7月-2014年7月我院ICU科收治的脓毒血症患者50例(脓毒血症组),一般感染患者40例(一般感染组),同时选取同期健康体检者30例作为正常对照组,采用酶联免疫吸附实验检测各组血清BNP水平,免疫比浊法检测各组血浆hs-CRP水平,超声心动图测量左房内径(LAD)、左室舒张末内径(LVEDD)、左室射血分数(LVEF)、短轴缩短率(FS)。结果:与正常对照组比较,一般感染组、脓毒血症组患者血清BNP、hs-CRP水平显著增高,差异有统计学意义(P均<0.01),而与一般感染组比较,脓毒血症组患者血清BNP、hs-CRP水平亦显著增高,差异有统计学意义(P均<0.01);单因素分析显示:BNP、hs-CRP均与LAD、LVEDD呈正相关(P均<0.05),与LVEF、FS呈负相关(P均<0.05),且BNP与hs-CRP亦呈显著的正相关(P<0.01)。结论:脓毒血症患者血浆BNP、hs-CRP水平均显著升高,并与心功能严重程度有良好的相关性,BNP、hs-CRP可作为判断脓毒血症心功能的良好指标。Objective:To explore the change and significance of Brain natriuretic peptide(BNP)and high sensitivity Creactive protein(hs-CRP)in patients with sepsis.Methods:Select the 50 patients with sepsis(sepsis group),40 patients with general infection(general infection group)and the 30 people with healthy subjects(control group)were chosen for the study.The levels of BNP were detected with by Enzyme-linked immunosorbent assay(ELISA);The levels of hsCRP were detected with by immune turbidimetry;The parameters of left atrial diameter(LAD),left ventricle end-diastolic diameter(LVEDD),left ventricle ejection fraction(LVEF),fraction shortening(FS)were detected with echocardiography.Results:The plasma BNP and hs-CRP in general infection group and sepsis group were significantly higher than the control group(All P〈0.01),but compared with the general infection group,sepsis group were significantly increased(P〈0.01).Correlation analysis revealed a strong correlation between the BNP with hs-CRP(r=0.945,P〈0.01);The levels of BNP and hs-CRP were positively correlated with LAD and LVEDD(All P〈0.01),and were negatively correlated with LVEF and FS(All P〈0.01).Conclusion:The concentration of BNP and hs-CRP were increased in patients with sepsis,which were closely associated with cardiac function.Measuring plasma BNP and hs-CRP levels may be helpful for judging the severity of cardiac function with sepsis.
