普瑞巴林联合多塞平窄谱中波紫外线治疗老年带状疱疹后神经痛疗效观察

摘　　要：目的观察普瑞巴林联合多塞平窄谱中波紫外线治疗老年带状疱疹后神经痛疗效及不良反应。方法 124例老年带状疱疹后神经痛患者随机分为观察组和对照组,每组62例,对照组患者给予多塞平口服和窄谱中波紫外线照射治疗,观察组在对照组治疗基础上给予普瑞巴林口服。结果治疗第4周时,观察组的总有效率达到98.3%,而对照组为80.6%,2组总有效率差异有统计学意义(P<0.05)。治疗第2周和第4周时,观察组的视觉模拟评分(VAS)显著低于对照组,差异有统计学意义(P<0.05)。观察组的止痛时间、疼痛消失时间均短于对照组,差异有统计学意义(P<0.01)。2组患者治疗后持续睡眠情况均显著好转,但观察组好转更明显(P<0.05或P<0.01)。观察组治疗后心理维度、生理维度、社会适应性以及生活质量总评分显著提高,与对照组之间差异有统计学意义(P<0.01)。2组患者不良反应主要为头晕、嗜睡,观察组患者不良反应发生率为6.4%,对照组为8.0%,2组比较差异无统计学意义(P>0.05)。结论普瑞巴林联合多塞平窄谱中波紫外线治疗老年带状疱疹后神经痛疗效确切,不良反应较低。Objective To observe the efficacy and the side effects of pregabalin and doxepin combined with narrow band-UVB in the elderly patients with postherpetic neuralgia. Methods 124 senile patients with postherpetic neuralgia were randomly divided into 2 groups： observation group and control group,with 62 cases in each group. Oral administration of doxepin combined with narrow band-UVB was given to the patients in both groups while oral administration of pregabalin was given additionally to patients in observation group. Results The total effective rate in observation group at the fourth week after the treatment was 98. 3%,compared with 80. 6% in control group（ P 0. 05）. The score of visual analogus scales（ VAS） of observation group at the second and the fourth week after the treatment were significantly lower than those in control group（ P 0. 05）. The analgensic time and the extinctive time of pain in observation group were shorter than those in control group with a statistical difference（ P 0. 01）. The continuous sleeping condition in both groups was obviously improved after treatment while the improvement in observation group was more significant（ P 0. 05 or P 0. 01）.The scores of psychological dimension,biological dimension,social adaptability in observation group were improved and the total score of life quality increased obviously after treatment,and there was significant difference between two groups（ P 0. 01）. The main side effects in the two groups were dizziness and sleepiness,and the incidence rate of side effect in observation group was 6. 4%,compared with 8. 0% in control group（ P 0. 05）. Conclusions The treatment with pregabalin and doxepin combined with narrow band-UVB is effective on postherpetic neuralgia in elderly patients with low side effects.
