老年2型糖尿病患者视网膜病变与血胱抑素C相关性研究

摘　　要：目的探讨老年2型糖尿病(T2DM)患者视网膜病变(DR)与血清胱抑素C(cystatin C,Cys-C)的相关性。方法选取我院2013年1月至2014年4月住院治疗的T2DM患者125例,进行眼底照相或眼底荧光造影,根据2002年国际分级标准将其分为增殖期DR(PDR)组35例、背景期DR(BDR)组41例、T2DM无DR病变(NDR)组49例,并选择同期老年健康对照(CON)组30例。分别检测各组空腹血糖(FBG)、Cys-C,糖化血红蛋白(Hb A1c)等指标,分析其相关性。结果 CON组与NDR组间Cys-C差异无统计学意义(P>0.05),与NDR组相比,PDR组及BDR组FBG、Cys-C、Hb A1c均显著升高,差异均有统计学意义(P<0.05),且随DR的进展呈增长趋势,PDR组与BDR组比较,Cys-C和Hb A1c差异均有统计学意义(P<0.05);相关分析显示FBG与Cys-C呈正相关(r=0.5,P<0.05),Hb A1c与Cys-C呈正相关(r=0.636,P<0.05)。结论 DR及其病情严重程度与血Cys-C密切相关,血Cys-C有可能成为预测老年DR发生发展的生物学指标。Objective To study the relationship between serum cystatin C and diabetic retinopathy（ DR） in the elderly patients with type 2 diabetes. Methods 125 cases of elderly patients with type 2 diabetes in our hospital from January 2013 to April 2014 were investigated to have a fundus examination or fundus fluorescence angiography. According to2002 international classification standard,all patients were divided into proliferative DR（ PDR） group（ n = 35）,background DR（ BDR） group（ n = 41）,no DR（ NDR） group（ n = 49）; 30 healty elderly volunteers were treated as control（ CON）group. The levels of fasting plasma glucose（ FBG）,Cys-C and Hb A1 c were determined and compared between the groups.Results Compared with CON group,the serum level of Cys-C of NDR group showed no statistical difference（ P 0. 05）.Compared with NDR group,the serum levels of Cys-C and FBG,Hb A1 c of PDR and BDR group were significantly increased（ P 0. 05） and the increase was more obviously in PDR group（ P 0. 05）. There were positive correlation between Cys-C and FBG,Hb A1c（ r = 0. 5,0. 636,P 0. 05）. Conclusions The severity of elderly patients with type 2 diabetic retinopathy is closely related with Cys-C. Cys-C may serve as a biological indicator to predict the occurrence and development of DR.
