三维方向性能量多普勒对胎儿脐带绕颈的检查及意义

摘　　要：目的探讨三维方向性能量多普勒(TDDPD)对胎儿脐带绕颈的诊断价值。方法 65例中期、68例晚期及71例足月妊娠的脐带绕颈单胎儿,分别采用TDDPD和三维彩色多普勒(TDCD)进行检查,比较脐带绕颈诊断准确率、脐带与胎盘血流关系及脐带附近软组织显示情况。结果 TDDPD对晚期和足月妊娠脐带绕颈的诊断准确率明显高于TDCD(P<0.05)。在各期妊娠中,TDDPD显示脐带与胎盘血流关系明显优于TDCD(P<0.05)。TDDPD可显示胎儿颈部及胎盘组织灰阶图像,而TDCD不能显示。结论 TDDPD对胎儿脐带绕颈的检测明显优于TDCD。Objective To investigate the diagnostic value of three-dimensional directional power Doppler（TDDPD） for umbilical cord around the neck（UCAN）. Methods TDDPD and three-dimensional color Doppler（TDCD） were used to detect 65 mid, 68 late and 71 full-term singleton pregnancies with UCAN. The diagnostic rate, umbilical cord-placenta blood stream, and soft tissue around the umbilical cord were compared. Results The diagnostic rates of TDDPD for UCAN were higher than those of TDCD in late and full-term pregnancies（P〈0.05）. The umbilical cord-placenta blood stream was more distinct using TDDPD compared with TDCD during all term pregnancies（P〈0.05）. TDDPD displayed the gray scale image of fetal neck and placental tissue, but TDCD did not. Conclusion TDDPD is superior to TDCD in the detection of UCAN.
