补阳还五汤对急性脑梗死患者预后状况及血清生长分化因子-15的影响

摘　　要：目的观察补阳还五汤对急性脑梗死(ACI)患者预后状况及血清生长分化因子-15(GDF-15)的影响。方法将90例急性期脑梗死患者随机分为观察组(45例)和对照组(45例)。对照组采用常规抗血小板聚集、改善循环治疗,观察组在对照组治疗基础上加服补阳还五汤。采用EISA法测定两组患者治疗前后血清GDF-15水平,以治疗前及治疗后1个月、3个月MRS量表评分为指标,比较两种治疗方法的疗效差异。结果两组治疗前血清GDF-15浓度与残疾程度(MRS)量表评分呈正相关(观察组:r=0.419,P=0.004;治疗组r=0.495,P=0.003);治疗后GDF-15水平同治疗组相比差异有统计学意义(P<0.05);观察组治疗后1个月、3个月总有效率分别为80%、84%;对照组分别为57%、62%,差异有统计学意义(P<0.05)。结论急性脑梗死患者血清GDF-15与疾病严重程度相关,补阳还五汤对急性期脑梗死患者的恢复有疗效,能显著降低血清GDF-15水平,提高急性脑梗死的治疗总有效率及远期疗效。Objective To investigate the effects of Buyang Huanwu decoction（BHD）on the prognosis and the levels of serum growth differentiation factor 15（GDF 15）in patients with acute cerebral infarction（ACI）.Methods Ninety patients with ACI were random-ly divided into two groups：The control group（n=45）treated with routine treatment,and the treatment group（n=45）treated with BHD plus routine treatment.The levels of serum GDF 15 were detected using ELISA pre and post treatment.Modified Rankin Scale（MRS）was used to evaluate the severity pre treatment and after 1 month and 3 months of treatment.Results The levels of serum GDF 15 were positively correlated with MRS scores post treatment （treatment group： r=0.419,P=0.004;control group： r=0.495,P=0.003）.There was a statistical y significant difference in serum GDF 15 levels between two groups post treatment （P〈0.05）.The total clinical effective rate were 80% and 84% in treatment group,which was significantly higher than that in control group after 1 month and 3 months of treatment（57% and 62%,P〈0.05）.Conclusion The serum GDF 15 level is correlated with the se-verity of ACI in patients with ACI.BHD can decrease the serum GDF 15 level and improve the prognosis in patients with ACI.
