红细胞体积分布宽度用于心肌梗死的预测价值分析

摘　　要：目的讨论红细胞体积分布宽度（RDW）对心肌梗死的预测价值分析。方法 2014年1月-2015年3月,对杭州市第一人民医院住院的120例心肌梗死患者和100例健康体检者测定RDW和常用心肌标志物（NT-pro BNP、Tn I、CK-MB）,采用SPSS 18.0进行统计学分析。结果心肌梗死组RDW值[（14.86±2.73）%]明显高于健康组[（12.80±0.47）%],差异具有统计学意义（P〈0.05）。经Pearson相关性分析,RDW与NT-pro BNP呈线性正相关（r=0.534,P〈0.05）;与Tn I和CK-MB无明显的相关性（P〉0.05）。另外,RDW值预测心肌梗死的受试者工作特征曲线下面积为0.884（95%CI：0.827-0.940）（P〈0.05）,临界值为13.5%,对应的预测灵敏度为76.8%,特异度为89.0%。结论RDW作为血细胞检验的常规参数,方便而廉价,能较好地预测心肌梗死的发病风险,是其他心肌标志物的有力补充。Objective To explore the predictive value of red cell volume distribution width（ RDW） in myocardial infarction.Methods RDW,and common cardiac markers（ NT-pro BNP,Tn I and CK-MB） of 120 patients with acute myocardial infarction who were admitted to the first people＇s hospital from January 2014 to March 2015,and 100 healthy examiners were detected. SPSS 18. 0 was used for statistical analysis. Results The RDW levels of myocardial infarction group were（ 14. 86 ±2. 73） %,which were significantly higher than those of the control group [（ 12. 80 ± 0. 47） %] and the difference was statistically significant（ P〈0. 05）. Pearson correlation analysis showed that RDW had positive linear correlation with NT-pro BNP（ r = 0. 534,P〈0. 05）; while no obvious correlation with Tn I and CK-MB. In addition,the area under receiver operating characteristics curve of predicton myocardial infractin of RDW level was 0. 884（ 95% CI： 0. 827-0. 940）（ P〈0. 05）; the corresponding cut-off point was 13. 5%,with corresponding sensitivity of 76. 8%,and specificity of 89. 0%. Conclusion RDW,as a routine parameter of blood cell test is an easy,inexpensive. It could be a good predictive marker for mlyocardial infraction,and it is a powerful complement to other cardic markers.
