联合检测CA125、CYFRA21-1及D-dimer对卵巢癌早期筛查与诊断的应用价值

摘　　要：目的观察联合检测糖类抗原(CA125)、细胞角蛋白19片段(CYFRA21-1)和D-二聚体(D-dimer)水平在卵巢癌早期筛查与诊断中的应用价值。方法采用化学发光法、免疫比浊法对48例卵巢癌患者以及44例卵巢良性肿瘤患者分别进行CA125、CYFRA21-1和D-dimer检测;以CA125、CYFRA21-1、D-dimer、CA125+CYFRA21-1和CA125+CYFRA21-1+D-dimer组合模式,进行敏感度、特异度、阴性预测值、阳性预测值及诊断效率的比较。结果单独检测CA125、CYFRA21-1、D-dimer的敏感度分别为68.2%、69.1%、64.4%;CA125+CYFRA21-1组合检测的敏感度为76.4%;CA125+CYFRA21-1+D-dimer组合检测的敏感度为91.2%,三项联合检测敏感度显著提高;单独检测CA125、CYFRA21-1、D-dimer的诊断效率分别为74.4%、73.8%、72.1%;CA125+CYFRA21-1组合诊断效率为81.7%;CA125+CYFRA21-1+D-dimer组合的诊断效率可达85.9%,三项联合检测诊断效率显著提高。结论CA125+CYFRA21-1+D-dimer联合检测能够显著提高临床对于卵巢癌早期筛查与诊断的效率。Objective To observe the clinical significance of joint detection of carbohydrate antigen 125（ CA125）,cytokeratin-19-fragment（ CYFRA21-1） and D-dimer for early screening and diagnosis of ovarian cancer. Methods 48 patients with ovarian cancer and 44 patients with benign tumor were collected. CA125,CYFRA21-1 and D-dimer were detected using chemical luminescence immune assay and turbidimetric inhibition immuno assay respectively. The sensitivity,specificity,negative predictive value,positive predictive value,diagnosis efficiency were compared among CA125 group,CYFRA21-1 group,D-dimer group,CA125 ＋ CYFRA21-1 group and CA125 ＋ CYFRA21-1 ＋ D-dimer group. Results The diagnosis sensitivity of CA125,CYFRA21-1 and D-dimer were 68. 2%,69. 1%,64. 4% respectively; that of the CA125 ＋ CYFRA21-1group and CA125 ＋ CYFRA21-1 ＋ D-dimer group were 76. 4% and 91. 2% respectively,the sensitivity of joint detection of CA125 ＋ CYFRA21-1 ＋ D-dimer was 91. 2%,which significantly improved. The diagnosis efficiency of CA125,CYFRA21-1and D-dimer were 74. 4%,73. 8%,72. 1%; that of CA125 ＋ CYFRA21-1 group and that of CA125 ＋ CYFRA21-1 ＋ D-dimer group were 81. 7% and 85. 9% respectively,and the diagnosis of joint detection of CA125 ＋ CYFRA21-1 ＋ D-dimer apparently improved. Conclusion Joint detection of CA125 ＋ CYFRA21-1 ＋ D-dimer group is conductive to early screening and diagnosis of ovarian cancer,which could play an important significance for early screening and diagnosis.
