标本的凝血和溶血对HIV/AIDS患者外周血CD4^+、CD8^+T淋巴细胞计数的影响

摘　　要：目的研究标本的凝血及溶血对HIV/AIDS患者外周血CD4+、CD8+T淋巴细胞计数的影响。方法选取HIV/AIDS患者30例,以抗凝全血、部分凝血、部分溶血血样分组,3组同时染色、裂解红细胞,流式细胞术检测CD4+、CD8+T淋巴细胞计数,对比3组结果差异。结果抗凝全血组CD4+T淋巴细胞数平均为(454.60±181.20)个/μl,部分凝血组为(294.33±132.75)个/μl,2组差异有统计学意义(P<0.05),部分凝血组CD8+、CD3+T淋巴细胞计数也明显减少,组间比较差异均有统计学意义(P<0.05);部分溶血组CD4+T淋巴细胞数平均为(505.47±182.05)个/μl,与抗凝全血组比较差异具有统计学意义(P<0.05),溶血组CD8+T淋巴细胞计数减少,2组比较差异具有统计学意义(P<0.05),但CD3+T淋巴细胞计数2组间比较差异无统计学意义(P>0.05)。结论血液凝固使CD4+、CD8+T淋巴细胞测定值减少,而溶血会增加CD4+T淋巴细胞测定值,减少CD8+T淋巴细胞计数。因此,检验人员应该加强责任心,规范化操作,采集合格的血液标本。Objective To study the influence of coagulated blood and hemolysis in samples on peripheral blood CD4^＋and CD8^＋T lymphocyte count in HIV / AIDS patients. Methods 30 patients with HIV / AIDS were selected for blood sample,which were grouped as blood anti-coagulation,partial coagulation and partial hemolysis,and they were stained simultaneously,the CD8^＋and CD4^＋T lymphocye were detected by splitting red cells and flow cytometry,and the results of the 3 groups were compared. Results The average CD4^＋T lymphocye in the blood anti-coagulation group was（ 454. 60 ± 181. 20） unit / μl,and that of the partial coagulation was（ 294. 33 ± 132. 75） unit / μl,and the difference between the two groups was statistically significant（ P〈0. 05）,the CD3＋and CD8^＋T lymphocye significantly decreased,and the difference was statistically significant（ P〈0. 05）; the average number of CD4^＋T lymphocye in partial hemolysis group was（ 505. 47 ± 182. 05） unit / μl,and the difference was statistically significant,compared with the blood anti-coagulation（ P〈0. 05）,the CD8^＋T lymphocye count decreased,the difference between the two groups was statistically significant（ P〈0. 05）,but there was no statistical significance on the difference between the two groups（ P〉0. 05）. Conclusion CD4^＋,CD8^＋T lymphocyte were reduced by blood coagulation,and the CD4^＋T lymphocyte was increased by blood coagulation,and CD8^＋T lymphocye decreased. Therefore,inspection personnel should strengthen the responsibility,standardize operation,and collect qualified blood samples.
