摘  要:目的探讨广西地区新生儿先天性甲状腺功能减退症(CH)筛查检出率及其与新生儿出生体重、孕周的关系。方法 2014年广西壮族自治区新生儿疾病筛查中心共筛查258 098名新生儿,出生后72 h采新生儿足跟血制作滤纸片标本,用时间分辨荧光免疫分析法检测血斑中促甲状腺激素(TSH)浓度,并将初筛值大于筛查切值者召回进行确诊检查。结果确诊CH患儿122例,发病率为1∶2 116。单一性早产儿或单一性低体重儿CH发病率和其他对照组新生儿比较差异均无统计学意义(P>0.05),只有在早产和低体重同时存在的新生儿中CH发病率会升高,发病率为1∶657,远高于其他新生儿(P<0.05)。结论早产低体重儿各方面系统功能不健全可能是造成CH高发病率的原因,建立完善的新生儿疾病筛查质量保障体系至关重要。Objective To explore the relationship between screening results of neonatal congenital hypothyroidism (CH) and birth weight, gestational weeks of neonates in Guangxi area in 2014. Methods A total of 258 098 neonates were screened in Neonatal Diseases Screening Center of Guangxi in 2014. Neonatal heel blood samples were obtained within 72 hours after birth to prepare dried blood spot filter paper, time-resolved fluorescence immunoassay was used to detect thyroid-stimulating hormone (TSH) concentrations, the neonates with preliminary screening values of TSH more than the cut-off value of TSH were recalled for definite diagnosis. Results A total of 122 neo- nates were diagnosed as CH definitely, the incidence rate was 1 : 2 116. There was no statistically significant difference in the incidence rate of CH between premature infants or low birth weight infants and the neonates in other control groups ( P〉0.05 ), the incidence rate of CH in- creased only among the neonates with premature birth combined with low birth weight, the incidence rate was 1 : 657, which was statistically significantly higher than those among the other neonates (P〈0. 05 ) . Conclusion The incompetence of body system of premature low birth weight infants may be the reason of high incidence rate of CH, it is important to establish perfect security system of neonatal diseases screening quality.
