摘　　要：目的探讨阻抗心动描记法(impedance cardiography,ICG)对感染性休克患者预后的评价作用。方法选择收治的75例感染性休克患者作为研究对象,按住院28d转归情况分为存活组和死亡组,比较2组血流动力学参数、动脉血乳酸、乳酸清除率、急性生理和慢性健康(Acute Physiology and Chronic Health Evaluation,APACHEⅡ)评分,研究这些因素与预后的关系。结果存活组乳酸清除率、左心室射血分数、心排出量、心脏指数、每搏排出量、心搏容量指数、加速指数高于死亡组,APACHEⅡ评分低于死亡组(P<0.05),乳酸清除率、左心室射血分数为影响感染性休克预后的独立危险因素(OR=0.552、0.769,P<0.05)。结论 ICG法能对感染性休克患者预后进行评价。Objective To evaluate the effects of impedance cardiography on the prognosis of patients with septic shock.Methods The clinical data of seventy-five patients with septic shock were divided into survival group and death group in accordance with prognosis in twenty-eight days.The hemodynamics,arterial blood lactic acid,lactate clearance and Acute Physical and Chronic Heath Evaluation Ⅱ（APACHEⅡ）scores were observed and the relationship between these factors and prognosis were evaluated.Results The lactate clearance,left ventricular ejection fraction,cardiac output,cardiac index,stroke volume,stroke volume index and acceleration index for the patients in survival group were significantly higher than those in death group,on the other hand,APACHEⅡ scores were lower than those in the death group（P〈0.05）.Lactate clearance and left ventricular ejection fraction were independent risk factors（OR=0.552,0.769,P〈0.05）.Conclusion Impedance cardiography can evaluate the prognosis of patients with septic shock.
