摘 要:目的了解石家庄市肺结核耐药情况,为制定适合本地情况的结核病控制策略提供科学依据。方法对全市2012-2013年所有涂片阳性肺结核患者的痰标本进行痰培养,对培养阳性的菌株进行菌株鉴定和药敏试验。结果共收集861份痰涂片阳性的痰标本,培养阳性795份(92.33%),进行药敏试验中发现结核分枝杆菌耐药183份,总体耐药率为24.27%,初、复治肺结核患者耐药分别为100份和83份,耐药率分别为16.64%和54.25%。发现耐多药肺结核和广泛耐多药肺结核患者分别为62例和9例,耐药率分别为8.22%和1.19%;年龄组初复治患者耐药经统计学分析,差异有统计学意义(χ2=11.184,P<0.05)。结论规范患者服药管理、提高服药完成治疗率、减少复发是做好耐药肺结核病防治工作的前提条件。Objective To study the drug- resistance of pulmonary tuberculosis in Shijiazhuang,and provide a scientific basis for the control strategies suitable for the local conditions. Methods All registered sputum smear- positive patients were collected in Shijiazhuang from 2012 to 2013. Culture positive strains for strain were identified and tested drug sensitive. Results There are 861 cases of patients with smear- positive sputum specimens,795 cases were cultured positive( 92. 33%). 183 cases were drug resistance of Mycobacterium tuberculosis,total drug resistance rate was 24. 27%. 100 cases were drug resistance of the primary treatment of patients,the resistant rate was 16. 64%. 83 cases were retreatment of patients,the resistant rate was 54. 25%.62 cases were MDR and resistance rate was 8. 22%. There are 9 cases of XDR and resistance rate was 1. 19%.Male,female patients with drug resistance was not statistically significant( χ2= 11. 184,P〈0. 05). Conclusions Improves the rate of completion treatment,reduces the recurrence of pulmonary tuberculosis. Those are the precondition for prevention and control of drug resistance tuberculosis.
