人文护理联合体位改变对疤痕子宫阴道分娩的影响

摘　　要：目的探讨人文护理联合体位改变对疤痕子宫阴道分娩的影响。方法随机把200例合并疤痕子宫孕妇在阴道试产中出现胎位异常的患者分为研究组120例和对照组80例,研究组实施人文护理联合体位改变,对照组实施体位改变。采用症状自评量表(SCL-90)来评估产妇分娩过程中的精神因素变化;对比分析分娩中产程出血量、产程时间、Apgar评分、新生儿窒息率、阴道分娩率、产钳助产率;对比分析产后并发症的发生率。结果实施人文护理干预后,研究组分娩过程中人际关系敏化、焦虑、抑郁症状明显降低(P<0.05);阴道分娩率、Apgar评分明显增高(P<0.05),产程时间、产程出血量、产钳助产率显著降低(P<0.05);产褥病率、产后感染、产后出血明显降低(P<0.01)。结论人文护理联合体位改变能显著降低疤痕子宫孕妇出现胎方位异常患者的焦虑、抑郁症状,提高阴道分娩率,降低阴道助产率,减少产后并发症。Objective The aim of this study is to investigate the effects ofhumanistic care combination with position change on the vaginal delivery of pregnant patients with scar uterus. Methods Randomly select 200 cases of pregnant patients that posed scar uterus and encountered abnormal fetal position when they were in their labor,120 cases of research group received humanistic care and position change,and 80 cases of control group were given position change.Self- reporting Inventory（ SCL- 90） was used to observe the spirit state of all the patients. The condition of labor including labor time,hemorrhage of labor,Apgar score,neonatal asphyxia rate,vaginal delivery rate,obstetrical forceps aid rate were compared between the two groups. The complications of postpartum,such as puerperal mobility,puerperal infection,postpartum hemorrhage,late postpartum hemorrhage,postpartum depression,puerperal sunstroke were researched. Results The patients received humanistic care,their symptoms of sensibilization of interpersonal relationship,anxiety,depressionwere significant improved,and there were significantly difference between the two groups（ P 0. 05）,accompanied with vaginal delivery rate,Apgar score enhanced（ P 0. 05）,and labor time,labor hemorrhage,obstetrical forceps aid rate,puerperal mobility,puerperal infection,postpartum hemorrhage decreased（ P 0. 05）. Conclusion Humanistic care combination with position change could improve the sprit state of patients who posed scar uterus and encountered abnormal fetal position in their labor progress,enhance vaginal delivery rate,low vaginal aid in labor,improve vaginal delivery rate.
