摘 要:目的分析维生素D3及其受体与稽留流产的免疫学相关性。方法选择2013年3月~2014年9月在深圳市罗湖区妇幼保健院妇产科接受治疗的稽留流产患者60例作为观察组,选择同期接受人工流产的正常女性60名作为对照组,比较两组受试者的血清维生素D3水平、Th1和Th2细胞含量、Th1和Th2细胞因子水平,分析维生素D3与稽留流产的免疫相关性。结果观察组患者的血清维生素D3水平[(12.89±1.56)nmol/L]明显低于对照组[(19.96±2.78)nmol/L],差异有高度统计学意义(P〈0.01);观察组患者Th1含量、Th1/Th2比例高于对照组[(11.41±1.34)比(7.64±0.87),(2.11±0.28)比(0.96±0.11)],Th2含量低于对照组[(5.42±0.62)比(7.95±0.91)],差异有统计学意义(P〈0.05);观察组患者的Th1因子IL-2、IFN-γ水平高于对照组[(78.12±9.14)比(28.65±4.22)ng/L,(56.67±6.91)比(29.45±4.68)ng/L],Th2因子IL-4、IL-10水平低于对照组[(66.48±7.92)比(109.54±13.16)ng/L,(18.31±2.14)比(33.46±5.13)μg/L],差异有统计学意义(P〈0.05);维生素D3水平与Th1因子IL-2、IFN-γ水平呈负相关(β=-1.712,r^2=0.452,t=12.122,P〈0.05;β=-1.083,r^2=0.509,t=14.632,P〈0.05),与Th2因子IL-4、IL-10水平呈正相关(β=1.452,r^2=0.551,t=9.329,P〈0.05;β=1.228,r^2=0.474,t=11.417,P〈0.05)。结论稽留流产孕妇存在维生素D3缺乏、Th1/Th2细胞向Th1偏移,且维生素D3含量与Th1、Th2细胞因子存在良好的相关性。Objective To study the correlation between vitamin D3 and receptor and immune of missed abortion. Methods 60 cases of patients with missed abortion from March 2013 to September 2014 in Shenzhen Luohu Maternal and Child Heatlth Hospital were enrolled as observation group, 60 cases normal female received induced abortion over the same period were enrolled as control group. Then serum vitamin D3 levels, Th1 and Th2 contents, Th1/Th2 factor levels were compared, correlation between vitamin D3 and immune function were compared. Results Vitamin D3 level of observation group [(12.89±1.56)nmol/L] was significantly lower than that of control group[(19.96 ±2.78)nmol/L](P〈0.01);and Th1, Th1/Th2 levels of observation group were significantly higher than those of control group [(11.41 ±1.34) vs(7.64±0.87),(2.11±0.28) vs(0.96±0.11)]; Th2 levels were significantly lower than those of control group [(5.42±0.62) vs(7.95±0.91)](P〈0.05); Th1 factor IL-2, IFN-γ levels of patients in observation group were higher than the control group [(78.12±9.14) vs(28.65±4.22)ng/L,(56.67±6.91) vs(29.45±4.68) ng/L], Th2 factor IL-4, IL-10 level was lower than the control group[(66.48±7.92) vs(109.54±13.16)ng/L,(18.31 ±2.14) vs(33.46±5.13) μg/L](P〈0.05); vitamin D level was negatively correlated with Th1 factors IFN-γ, IL-2 levels(β=-1.712, r^2=0.452, t=12.122, P〈0.05; β=-1.083,r2=0.509, t=14.632, P〈0.05); and positively correlated with Th2 factors IL-4, IL-l0 levels(β=1.452, r^2=0.551, t=9.329,P〈0.05;β =1.228, r^2=0.474, t =11.417, P〈0.05). Conclusion Missed abortion pregnant women presence vitamin D3 deficiency, Th1/Th2 cells shift to Th1; vitamin D3 levels have good correlation with Th1/Th2 factor levels.
