结核抗体试验与TSPOT-TB试验诊断肺结核的临床价值比较

摘　　要：目的比较结核抗体试验与TSPOT-TB试验对肺结核病的诊断价值。方法选取2014年1月-2015年1月我院收治的疑似肺结核病患者95例,分别给予结核抗体试验与TSPOT-TB试验检测,比较两者诊肺断结核的敏感性、特异性、阳性预期值、阴性预期值。结果确诊为肺结核72例,TSPOT-TB试验法敏感性低于结核抗体试验法,χ^2=1.25,P〉0.05,差异不具有统计学意义;TSPOT-TB试验法阴性预期值高于结核抗体试验法,χ^2=1.65,P〉0.05,差异不具有统计学意义;TSPOT-TB试验法的特异性、阳性预期值分别为82.61%、92.45%,高于结核抗体试验法,χ^2=12.74、χ^2=5.04,P〈0.05,差异具有统计学意义。结论 TSPOT-TB试验法具有高敏感性、特异性、阳性预期值高特点,是结核筛查的有效试验方法。Objective Compare the clinical value of tuberculosis antibody test and TSPOT-TB test for diagnosis of pulmonary tuberculosis.Methods Selected 95 cases of suspected tuberculosis patients from January 2014 to January 2015 in our hospital, Use respectively tuberculosis antibody test and TSPOT-TB test, The sensitivity, speciifcity, positive and negative predictive value of the diagnosis of pulmonary tuberculosis were compared. Results 72 cases were diagnosed as pulmonary tuberculosis. The sensitivity of TSPOT-TB test was less than tuberculosis antibody test, χ^2=1.25,P〉0.05, had no difference statistically signiifcance, The negative predictive value of TSPOT-TB test was higher than tuberculosis antibody test, χ^2=1.65, P〉0.05, had no difference statistically significance, The specificity and the positive predictive value of TSPOT-TB test were 82.61% and 92.45% higher than that of tuberculosis antibody test, χ^2=12.74, χ^2=5.04,P〈0.05, had no difference statistically signiifcance.Conclusion TSPOT-TB test is high sensitive , speciifc, high positive expected value,and it is an effective method for the screening of tuberculosis .
