伴有2型糖尿病的老年性急性阑尾炎52例治疗体会

摘　　要：目的通过对本院治疗的伴有2型糖尿病的老年性急性阑尾炎患者进行回顾性研究,对早期诊断、治疗方法进行研究。方法选取我院2008年1月-2014年12月收治的年龄在60岁以上,明确诊断为2型糖尿病的急性阑尾炎患者52例,其中手术治疗36例,非手术治疗16例。结果手术治疗组36例,29例治愈,1例出现切口裂开,经二次缝合切口治愈出院,6例出现术后切口感染,加强切口引流换药后治愈出院;非手术治疗组16例,9例治愈,2例有效,5例无效转手术治疗。结论伴有2型糖尿病的老年性急性阑尾炎是一种特殊的阑尾炎类型,一旦确诊就应尽早行手术治疗,术中充分冲洗引流,术后积极抗感染治疗,尽量减少并发症。ObjectiveRetrospective study was carried out through to the hospital treatment of acute appendicitis complicated with type 2 diabetes mellitus senile patients for early diagnosis, treatment, research and discussion.Methods Selected 52 cases who had years of age above 60 with senile acute appendicitis accompanied with type 2 diabetes from January 2008 to December 2014, 36 cases of surgical treatment and 16 cases of the non operative treatment.Results The 36 cases of surgical treatment, 29 cases were cured, 1 case had wound dehiscence, after two times of suturing incisions was cured, incisional infection occurred in 6 cases, incisionand drainage after cured. The 16 cases of the non operative treatment, 9 cases were cured, 2 cases were effective, 5 cases turn operation treatment. Conclusion Senile acute appendicitis accompanied with type 2 diabetes is a special type of appendicitis, once diagnosed early surgical treatment should be completely lfushed, intraoperative and postoperative drainage actively anti-inlfammatory treatment, try to reduced the complications.
