摘 要:目的分析输尿管硬镜手术中进镜困难的原因以及处理方法,以提高输尿管硬镜的技术水平。方法选取我院输尿管中下段结石行输尿管镜钬激光碎石取石术患者852例,其中93例术中存在不同程度进镜困难,对其原因进行分析。结果针对不同的原因采用相应方法进镜,成功89例,失败4例,改开放手术1例,3例留置输尿管双J管后二期输尿管镜手术。结论输尿管进镜困难的原因主要为输尿管开口或壁内段狭窄等,熟练掌握相关技巧可以提高手术成功率。Objective To explore the causes and treatment of dififculties in accessing into ureter during rigid ureteroscopy, for improveing the technical level.Methods Selected 852 cases underwent transurethral ureteroscopy, the data were evaluated retrospectively. 93 cases had difficulties in ureteroscope into ureter in operation, analyzed the reasons.Results Among 93 cases, 89 cases were completed successfully and 4 cases failed. 1 case was converted to open operation,3 cases were left with double J ureteral operation for the second surgery.ConclusionThe causes of dififculties in accessing into ureter during conventional adult ureteroscopy include the intramural ureteral stricture. The key to successful retrograde access to the rigid ureteroscope lies in the urologist′s operative skills.
