摘 要:目的探讨实验室检查血清降钙素原(PCT)水平对临床判断医院细菌感染的价值,为临床诊断医院细菌感染提供实验室测定方法。方法对2013年5月-2014年4月住院患者发生医院感染427例,将其分为非重症感染291例(A组)、重症感染136例(B组),同时选取非医院感染的住院患者277例作为对照组,通过半定量免疫色谱法测定PCT水平,通过高敏免疫比浊法测定超敏C-反应蛋白(hs-CRP)水平,通过血细胞分析仪测定白细胞(WBC)计数。结果 A、B和对照组患者PCT>10.0μg/L者分别为14.09%、67.65%和0,A组明显低于B组,但是明显高于对照组;A、B、对照组患者PCT阳性率分别为36.00%、89.71%、7.49%,A组明显低于B组、高于对照组,组间两两比较差异有统计学意义(P<0.05);A、B、对照组患者hs-CRP、WBC水平,A、B两组患者明显高于对照组(P<0.05),A、B两组之间比较差异无统计学意义。结论通过实验室测定PCT水平可以初步判断是否发生了医院细菌感染以及感染的严重程度,从而为早期采取治疗并判断患者预后提供依据。OBJECTIVE To explore the value of serum procalcitonin(PCT)level in clinical diagnosis of hospital-associated bacterial infections so as to offer the laboratory examination method for clinical diagnosis of the hospitalassociated bacterial infections.METHODS A total of 427 patients with hospital-associated infections who were hospitalized from May 2013 to Apr 2014 were enrolled in the study;291patients with non-severe infections were assigned as the group A,and 136 patients with severe infections were set as the group B,meanwhile,277 hospitalized patients without hospital-associated infections were chosen as the control group.The PCT level was determined by means of semi-quantitative immuno-chromatographic method,the high-sensitivity C-reactive protein(hsCRP)level was measured by using high-sensitivity immunoturbidimetric assay,and the white blood cell(WBC)counts were measured with the use of hematology analyzer.RESULTS The proportion of the patients with the PCT level more than 10.0μg/L was 14.09%in the group A,significantly lower than 67.65%in the group B,but significantly higher than 0in the control group;the positive rate of PCT of the group A was 36.00%,significantly lower than 89.71% of the group B,but significantly higher than 7.49% of the control group,and there was significant between- group difference(P〈0.05).The levels of hs-CRP and WBC of the group A and group B were significantly higher than those of the control group(P〈0.05),and there was no statistically significant difference between the group A and the group B.CONCLUSION The laboratory detection of the PCT level is conducive to the preliminary determination of the hospital-associated bacterial infections and the severity of the infections so as to provide guidance for early treatment and prediction of prognosis.
