摘　　要：目的探讨喉癌患者术后发生手术部位感染(SSI)的相关危险因素及护理对策,为临床预防提供理论基础。方法回顾性分析2010年7月-2013年11月在医院行喉切除术治疗83例喉癌患者临床资料,分析喉癌患者术后SSI的危险因素,并提出相应护理对策。结果 83例患者中术后发生SSI 12例,发生率为14.5%;单因素分析显示,感染患者临床Ⅲ分期以上、术前有气管切开、手术时间>4h、全喉切除等方面差异均有统计学意义(P<0.05);logistic多因素逐步回归显示,患者临床Ⅲ分期以上、术前有气管切开、手术时间>4h、全喉切除是术后发生SSI的独立危险因素(P<0.05)。结论喉癌术后SSI的危险因素与临床分期、全喉切除等指标有关,对相应因素予以有效的护理干预对策,可减少术后SSI发生率。OBJECTIVE To explore the related risk factors for postoperative surgical site infections（SSI）in the patients with laryngeal cancer and put forward the nursing countermeasures so as to provide theoretical basis for clinical prevention.METHODS The clinical data of 83 patients with laryngeal cancer who underwent the laryngectomy from Jul 2010 to Nov 2013 were retrospectively analyzed,the risk factors for the postoperative SSI were observed,and the corresponding nursing countermeasures were put forward.RESULTS The postoperative SSI occurred in 12 of 83patients,with the incidence rate of 14.5%.The univariate analysis indicated that there was statistically significant difference in the clinical stage aboveⅢ,preoperative tracheotomy,operation duration more than 4hours,or radical laryngectomy among the patients with the infections（P〈0.05）.The multivariate logistic stepwise regression analysis showed that the independent risk factors for the postoperative SSI included the clinical stage aboveⅢ,preoperative tracheotomy,operation duration more than 4hours,and radical laryngectomy（P〈0.05）.CONCLUSION The incidence of the postoperative SSI in the patients with laryngeal cancer is associated with clinical stage and radical laryngectomy.It is necessary to take effective nursing countermeasures so as to reduce the incidence of postoperative SSI.
