量化质量控制在医院感染预警化管理的应用

摘　　要：目的利用量化质量控制的手段,建立分级量化管理指标体系,使用警戒值和阈值对比临床科室的量化考核情况,实现对临床科室医院感染质量的警示和调控。方法该研究引入量化质量控制和预警化管理理念,通过专家咨询法,于2012-2013年建立医院感染管理三级量化考核指标体系;通过实证研究,设计出医院感染质量控制的警戒值和阈值,并利用该预警体制对医院2014年各临床科室进行考核评价,最终实现医院感染的预警化管理。结果建立三级量化指标考核体系,共计5个项目26个指标,将26个指标分成关键指标、重要指标以及一般指标;根据计算得出9个临床科室2012-2013年医院感染考核得分的月平均值在75~87分,总体均值为81.46分;质量控制平均警戒值为75.48分,质量控制平均阈值为73.59分;医院利用该警戒值和阈值对2014年各临床科室进行考核评价,及时发现并控制一起疑似医院感染暴发事件。结论利用量化质量控制的手段,可以对各科室医院感染质量及时予以警示和调控,预警化管理方式是医院感染风险管理的最佳表现模式。OBJECTIVE To use quantitative quality control method,establish the quantitative classification management index system,and adopt the warning value and threshold value for observation of quantitative assessment status in clinical departments so as to realize the forewarning management of nosocomial infections in the clinical departments.METHODS The idea of quantitative quality control management and forewarning management was introduced in the study,the three-level quantitative assessment index system of nosocomial infection management was established from 2012 to 2013through the expert consultation method.The warning value and threshold value were determined through empirical study,the forewarning system was used to conduct the assessment and evaluation of clinical departments in 2014,and the forewarning management of nosocomial infections was finally achieved.RESULTS The three-level quantitative index assessment system was established,involving 26 indicators in5projects;the 26 indicators were divided into the key indicators,important indicators,and general indicators.The monthly average score of assessment of nosocomial infections varied from 75 to 87points in the 9clinical departments from 2012 to 2013,with the total mean score of 81.46 points.The mean warning value of quality control was 75.48 points,and the mean threshold value of quality control was 73.59 points.The warning value and threshold value were used to conduct the assessment and evaluation of the clinical departments in 2014,and one case of nosocomial infections was detected and controlled.CONCLUSION The use of the quantitative quality control method can contribute to the timely forewarning and regulation of the nosocomial infection management quality.The forewarning management is the optimal manifestation mode of risk management of nosocomial infection.
