摘 要:目的探讨声速匹配(SSC)技术在不同大小乳腺病灶中的诊断价值。方法对经病理证实的483个乳腺肿物行SSC检查,获得其声速匹配值(ZSI)及声速(SV)。根据最大径将病灶分为≤2cm组和>2cm组。构建ROC曲线,确定临界值。结果总样本组、≤2cm组与>2cm组良性病灶及恶性病灶的SV差异均有统计学意义(P均<0.05);3组曲线下面积(AUC)分别为0.875、0.862、0.889,两两比较差异无统计学意义(P均>0.05);以1560.00m/s作为SV临界值,3组的敏感度分别为81.15%(99/122)、79.73%(59/74)、83.33%(40/48);特异度分别为90.03%(325/361)、90.20%(267/296)、89.23%(58/65);准确率分别为87.78%(424/483)、88.11%(326/370)、86.73%(98/113)。结论SSC在不同大小乳腺良恶性肿物的鉴别诊断中具有较好的临床价值,且诊断效能相似,SV临界值为1560.00m/s。Objective To investigate the diagnostic value of sound speed correction(SSC)technique for breast lesions with different size.Methods Four hundred and eighty-three pathologically proved lesions were included.By the SSC technique,the zone speed index(ZSI)and sound speed(SV)of the lesions were calculated.According to maximum diameter,the lesions were divided into≤2cm group and〉2cm group.The ROC curve was constructed to evaluate the cut-off point of SV in different groups.Results The SV in all lesions group,≤2cm group and 〉2cm group had statistically significant difference in benign or malignant breast lesions(all P〈0.05).The area under curve(AUC)of ROC was 0.875,0.862,0.889,respectively.There was no significant difference between the AUC of any two groups(all P〈0.05).The cut-off point of SV was determined as 1560.00m/s,with the sensitivity of 81.15%(99/122),79.73%(59/74),83.33%(40/48),the specificity of 90.03%(325/361),90.20%(267/296),89.23%(58/65),and the accuracy of 87.78%(424/483),88.11%(326/370),86.73%(98/113),respectively.Conclusion SSC technique has a good and similar clinical value for diagnosis of benign and malignant breast lesions in different size with a cut-off point of 1560.00m/s.
