摘　　要：目的探讨胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP-3)与新生儿出生体重及巨大儿出生率的关系。方法应用ELISA法测定85例正常妊娠(正常组)、80例妊娠糖尿病组(GDM组)及80例胎儿宫内发育迟缓(IUCR组)足月分娩时母血及脐血中IGF-1、IGFBP-3水平,观察三组新生儿出生体重,并分析IGF-1、IGFBP-3与巨大儿的关系。结果正常组、GDM组及IUCR组母血IGF-1、IGFBP-3水平无统计学差异(P>0.05)。GDM组脐血中IGF-1、IGFBP-3水平显著高于正常组及IUCR组(P<0.05),而正常组脐血中IGF-1、IGFBP-3水平显著高于IUCR组(P<0.05)。GDM组新生儿出生体重显著重于对照组及IUCR组(P<0.05),而巨大儿出生率高于对照组及IUCR组(P<0.05)。与低出生体重儿及正常体重儿相比,巨大儿脐血中IGF-1、IGFBP-3水平显著较高(P<0.05),而正常体重儿脐血中IGF-1、IGFBP-3水平高于低出生体重儿(P<0.05)。结论脐血中IGF-1、IGFBP-3水平对胎儿宫内发育具有重要的调节作用,可能是胎盘中影响胎儿生长发育的重要因子,并与IUGR及巨大儿形成有密切的关系。Objective：To investigate the relationship of the levels of insulin-like growth factor-1（IGF-1）,insulin-like growth factor binding protein 3（IGFBP-3）.Methods：The levels of IGF-1,IGFBP-3 of 85 cases of normal pregnancy（control group）,80 cases of gestational diabetes mellitus（GDM group）and 80 cases of intrauterine growth retardation（IUCR group）were detected with ELISA.The relationship of the levels of insulin-like growth factor-1（IGF-1）,insulin-like growth factor binding protein 3（IGFBP-3）were analyzed. Results：The levels of maternal IGF-1,IGFBP-3 of normal group,GDM group and IUCR group were not statistically significant（P〈0.05）.The levels of cord blood IGF-1,IGFBP-3 of GDM group were higher than the normal group and IUCR group（P〈0.05）,while the levels of cord blood of IGF-1,IGFBP-3 of normal group were higher than IUCR group（P〈0.05）.The birth weight of GDM groups were higher than control and IUCR group（P〈0.05）,while the birth rate was higher and IUCR huge group of children（P〈0.05）. Compared with low birth weight children and normal weight children,great children cord blood IGF-1,IGFBP-3levels were significantly higher（P〈0.05）,while the normal weight children cord blood IGF-1,IGFBP-3 levels higher than the low-birth-weight children（P〈0.05）.Conclusion：The cord blood IGF-1,IGFBP-3,LEP levels on fetal development has an important regulatory role,may be an important factor in placenta affect fetal growth and development,and formed a close relationship with IUGR and great children.
