巨刺运动疗法治疗肩关节周围炎的临床研究

摘　　要：目的:比较巨刺运动疗法与常规针刺治疗肩关节周围炎的临床差异。方法:选取78例肩关节周围炎的患者,随机分组后分别采用常规针刺方法(常规组)和巨刺运动疗法(治疗组)治疗,同时配合肩部功能锻炼。于首次治疗前后、2个疗程后、末次治疗后(4个疗程为限)评估其目测类比评分(VAS)、肩关节严重指数(SSI)。结果:首次治疗后即刻两组患者VAS、SSI评分比较P<0.05,治疗组疗效优于常规组;经过2个疗程10次治疗,两组VAS、SSI评分比较P<0.05,治疗组的疗效优于常规组;经过4个疗程20次治疗,两组患者VAS评分比较P<0.05,治疗组优于常规组,而SSI评分比较P>0.05,两者差别不明显。结论:巨刺运动疗法在肩关节周围炎的治疗中缓解疼痛的作用明显优于常规针刺方法,在改善功能、提高肌力等方面尚需进一步验证。Objective： To compare the clinical differences between the two acupuncture methods for scapulohumeral periarthritis. Methods： 78 cases of patients with scapulohumeral periarthritis were treated by contralateral meridian needling plus movement or the conventional acupuncture treatment after randomization,in conjunction with the shoulder functional exercise. Before and after the first treatment,2 courses of treatment,after the last treatment（ 4 courses only） assess their visual analog scale（ VAS）,shoulder severity index（ SSI）. Results： Compared immediately after the first treatment,VAS,SSI scores had a significant difference between two groups（ P〈0. 05）,and the treatment group was better than the conventional group; after two courses,VAS,SSI scores still had a significant difference between two groups（ P〈0. 05）,and the treatment group was better than the conventional group; after four courses treatment,VAS still had a significant difference between two groups（ P〈0. 05）,while SSI scores had no significant difference（ P〉0. 05）. Conclusion： According to the scapulohumeral periarthritis,the treatment of contralateral meridian needling plus movement is superior to the conventional method in pain relieving,while further study is needed in improving function,increasing muscle strength and so on.
