腹腔镜精索静脉高位结扎术对精子质量及妊娠结局的影响

摘　　要：目的 探讨腹腔镜保留动脉精索静脉高位结扎术对精子质量的影响.方法 回顾分析33例我院2011年4月至2012年7月因不育就诊的精索静脉曲张患者,并行腹腔镜保留动脉精索静脉高位结扎手术,分别比较手术前后精液各参数变化,包括精液容积（mL）、精子浓度（×106/mL）、前向运动精子百分率（％）、前向运动精子总数（×106）等.结果 腹腔镜保留动脉精索静脉高位结扎术后精子浓度（29.20±26.69）×106/mL、前向运动精子百分率（31.33±16.21％）,前向运动精子总数（34.81±36.19）×106与术前精子浓度（13.72±17.43）× 106/mL、前向运动精子百分率（17.06±10.25）％,前向运动精子总数（8.06±8.42）×106比较,均有统计学意义（P＜0.05）,其中15例手术后1年中女方怀孕,妊娠率为48.48％.术后精液容积（3.80±1.24）mL与术前相比（3.66±1.60） mL差异无统计学意义（P＞0.05）.结论 腹腔镜保留动脉精索静脉高位结扎术可以明显改善不育伴精索静脉曲张患者的精子浓度、前向运动精子百分率,前向运动精子总数,而精液容积无改善.48.48％不育症患者手术后1年内可使女性正常怀孕.Objective To study the impact of laparoscopic with keeping artery of spermatic vein high ligation on the quality of sperm. Methods Thirty±three patients with varicocele infertility in our hospital from April 2011 to July 2012 who underwent laparoscopic with keeping artery of Spermatic vein high ligation were retrospectively analyzed, and the changes in semen parameters before and after operation including the semen volume （mL）, sperm concentration （× 106/ mL）, the forward movement of sperm percentage（%） and the total number of sperm forward movement （× 106） etc were comparatively analyzed. Results After treament, their sperm concentration （29.204±26.69 × 106/mL, the forward movement of sperm percentage （31.334±16.21）% and the total number of sperm forward movement （34.81±36.19）× 106 had all statistical significance as compared with preoperative sperm concentration （13.724±17.43）× 106/mL, the forward movement of sperm percentage（17.064±10.25）% and the total number of sperm forward movement （8.06±8.42）× 106. Fifteen patients＇ wife was pregnant within 1 year after the operation and the pregnancy rate was 48.48%. There was no significant difference in semen volume between postoperation （3.804±1.24）mL and preoperation （3.664±1.60）mL. ConclusionLaparoscopic with keeping artery of spermatic vein high ligation can greatly improve the sperm concentration,the forward movement of sperm percentage and the total number of sperm forward movement of the sterility of varicocele, but no effect on semen volume. About 48.48% infertility patients within 1 year after the operation can impregnate their spouses.
