益气补肾中药与雷公藤多苷联合缬沙坦治疗脾肾气虚型糖尿病肾病的随机对照研究

摘　　要：目的：本研究通过观察中药（雷公藤多苷片＋中药汤剂）合用ARB与单用ARB对脾肾气虚型糖尿病肾病患者的临床疗效以探讨中药联合ARB治疗本病的优势。方法：将60例脾肾气虚型2型DNⅢ-Ⅳ期患者随机分成治疗组和对照组各30例。对照组在控制饮食、血压、血糖、血脂等基础上予缬沙坦胶囊治疗,治疗组在对照组基础上予加用益气补肾中药联合雷公藤多苷片,疗程1年。结果：经治疗后两组患者血糖、血脂均显著下降,治疗组血脂水平更低于对照组;两组尿蛋白均显著下降,且治疗组好于对照组;两组患者治疗期间均未见明显副作用。结论：益气补肾中药与雷公藤多苷联合缬沙坦治疗脾肾气虚型糖尿病肾病能减少尿蛋白量、降低血脂、延缓糖尿病肾病进展,较单用ARB制剂缬沙坦更为有效。Objective： The effects of Combination Of Yiqibushen decoction with Tripterygium Glycosides on the basis of valsartan for the treatment of Diabetic Nephropathy remains unclear. This study was performed to clarify the effects of Traditional Chinese medicine on Diabetic Nephropathy by comparing the combination of the traditional Chinese medicine（ yiqibushen Medicine and tripterygium glycosides） and valsartan to valsartan alone. Methods： A total of 60 subjects were randomly divide（ Spleen and kidney deficiency type） with Phase Ⅲ- Ⅳ DN into control group and treatment group with 30 cases in each. The control group patients were under treament of the control of diet,blood pressure,serum glucose and lipid as well as valsartan capsule,the treatment group patients were received the Yiqibushen decoction and tripterygium glycosides tablets on the basis of the therapy of the control group. The patients were followed up for 1 year. Results： Serum glucose and lipid were significantly decreased both groups at the end of the follow- up. Serum lipid was lower in the treatment group than that in the control group. Proteinuria was decreased in both groups. Furthermore,Proteinuria in the treatment group was significantly reduced compared with the control group. The effective rates of the control and the treatment group were 86. 7%（ 26 /30） and 63. 4%（ 19 /30）,respectively. The effective rate between the two groups was significant different（ P〈0. 05）. Conclusion： The combination of yiqibushen decoction with tripterygium glycosides tablets and valsartan could significantly reduce urinary proteinuria and serum lipid to delay the progress of DN with Spleen and kidney deficiency which was superior to valsartan therapy alone.
