摘  要:目的探讨乙酰半胱氨酸对肺纤维化合并肺气肿(combined pulmonary fibrosis and emphysema,CPFE)综合征患者氧化应激指标和肺功能的影响。方法将112例CPFE综合征患者随机分为处理组与对照组,各56例,两组都给予西医常规抗炎、支气管扩张剂及氧疗等基本治疗,处理组除了给予基本治疗,还给予口服乙酰半胱氨酸治疗。抽取受试者早晨空腹静脉血,运用化学比色法测定两组患者治疗前后血MDA、GSH-Px和SOD含量,并且比较两组患者治疗前后FEV1、FVC、DLCOSB。结果处理组治疗后与对照组治疗后比较MDA明显下降(P<0.01),GSH-Px和SOD明显升高(P<0.05,P<0.01),处理组患者治疗后FEV1、FVC、DLCOSB的改善情况均优于对照组(P<0.05,P<0.01)。结论 N-乙酰半胱氨酸用于治疗CPFE综合征具有抗氧化、改善肺功能(尤其是肺的弥散功能)的作用。Objective To investigate the influence of N-acetylcysteine( NAC) in the treatment of combined pulmonary fibrosis and emphysema syndrome index of oxidative stress and pulmonary function.Methods Atotal of 112 case of CPFE syndrome admitted in our hospital from January 2013 to December2014 were randomly divided into 2 groups: the treatment group and the control group,56 cases in each. Two groups were given conventional anti-inflammatory, bronchodilator and oxygen therapies and other basic therapy, and the treatment group received oral NAC treatment in addition. The contents of malondialdialdehyde( MDA) glutathione peroxidase superoxide dismutase( GSH-Px) and( SOD) of 2 groups of patients before and after treatment were detected by chemical colorimetry,and forced expiratory volume 1second( FEV1), forced vital capacity( FVC), single breath diffusing capacity for carbon monoxide( DLCOSB) before and after treatment their were compared. Results The levels of MDA in blood plasma of the treatment group Significantly lower than the control group after treatment,SOD and GSH-Px levels were significantly higher,the improvement of FEV1,FVC and DLCOSB in treatment group was better than that in control group. Conclusion N-acetylcysteine treatment of CPFE syndrome patients can not only exert antioxidant effect, but also can improve the pulmonary function of patients, especially the pulmonary dispersion function.
