摘  要:目的分析检测不同分期慢性肾脏病(chronic kidney disease,CKD)患者骨代谢标志物水平的临床意义。方法选择CKIM期患者40例、CKD5期患者84例(透析患者35例,非透析患者49例),分别检测4种血清骨标准物(血清骨钙素、I型胶原羧基端肽8特殊序列、甲状旁腺素、总I型前胶原氨基端延长肽)水平。结果随着慢性肾脏病病情进展,4种骨代谢标志物水平逐渐升高,CKD4期患者均低于CKD5患者,差异有统计学意义(P〈0.01);4种血清骨代谢标志物水平CKD5期透析与非透析患者差异无统计学意义(P〉0.05)。结论检测骨代谢标志物水平能早期、敏感反映肾性骨病的发展及变化,为后续诊断及治疗提供依据。Objective To investigate the changes of bone metabolic markers in patients with Chronic Kidney Disease (CKD). Mothods The serum levels of osteocalcin telopeptide (OSTEOC), C - telopepfides of type I collagen(β - CTX), parathyroidhormone (PTH)and total procollagen type 1 N- terminal peptide (TPINP)were measured in 124 chronic kidney patients. Results The levels of serum bone metabolic markers were increased with the development of CKD. The levels of serum OSTEOC,β- CTX, PTH and TPINP were higher in the CKD5 patients than in the CKD4 patients( P 〈 0.05) .The levels of the four bone metabolic markers were no difference between CDK5 dialysis patients and CDK5 non - dialysis patients. Conclusion The levels of serum bone metabolic markers may represent the development and changes of bone metabolism in the CKD patients.This may provide basis for the diagnosis and treatment of renal osteopathy disease.
