加味苓甘五味姜辛半夏汤治疗婴幼儿哮喘45例临床观察

摘　　要：目的:观察加味苓甘五味姜辛半夏汤治疗婴幼儿哮喘的有效性和安全性及其对患儿肺功能与Clara细胞分泌蛋白的影响。方法:将90例哮喘患儿随机分为两组,治疗组45例予以加味苓甘五味姜辛半夏汤治疗,对照组45例予以雾化吸入布地奈德、复方异丙托溴铵联合抗感染治疗。两组均治疗5d后观察疗效。结果:愈显率治疗组为82.22%,对照组为55.56%,组间比较,差异有统计学意义(P<0.01);在缓解咳嗽、喘息、肺部哮鸣音方面两组差异无统计学意义(P>0.05),但两组喉中痰鸣与肺部痰鸣音改善比较,差异有统计学意义(P<0.01);且治疗组在Clara细胞分泌蛋白调节、2周后复查肺功能FEV1%和PEF%方面,较对照组均有明显改善(P<0.05)。结论:加味苓甘五味姜辛半夏汤治疗婴幼儿哮喘安全有效,可能系通过内源性兔疫调节,获得温肺化痰、降逆平喘之功效。Objective： To observe the efficacy and safety of modified Linggan Wuwei Jiangxin Banxia decoction for treatment of infantile asthma and its impacts on lung function and Clara cell secretory protein. Methods： Ninety infants with asthma were randomly and equally divided into treatment group and control group. Patients in the treatment group were given modified Linggan Wuwei Jiangxin Banxia decoction,while patients in the control group received anti- infection therapy with budesonide and compound ipratropium bromide for aerosol inhalation. The treatment outcomes were evaluated after 5 days of treatment. Results： The cure and marked response rate in the treatment group was significantly higher than that in the control group（ 82. 2% vs55. 56%,P 〈0. 01）. There were no significant differences in relief of cough,dyspnea,or wheezing between the two groups（ P 〉0. 05）. However,compared with the control group,the treatment group had significant improvement in wheezy phlegm in the throat and lung（ P 〈0. 01）. Moreover,the treatment group had significant improvements in regulation of Clara cell secretory protein and FEV1 and PEF at 2 weeks after treatment compared with the control group（ P〈 0. 05）. Conclusion： Modified Linggan Wuwei Jiangxin Banxia decoction is safe and effective in the treatment of infantile asthma. This therapy can warm the lungs for resolving phlegm and lower adverse qi for relieving asthma by regulation of the innate immune system.
