室壁瘤形成过程中基质金属蛋白酶-9与心功能及室壁瘤容积相关性研究

摘　　要：目的探讨室壁瘤形成过程中基质金属蛋白酶-9(MMP-9)血清浓度与心功能及室壁瘤容积的相关性。方法 20只新西兰大白兔结扎左前降支及左回旋支制作室壁瘤模型。于术前,术后1、2、3d,术后1、2、3、4周进行常规超声心动图检查,同时耳缘静脉抽血,至术后4周有室壁瘤形成的实验兔纳入研究。使用实时三维超声心动图测量左心室容积、左心室射血分数(LVEF)及室壁瘤容积(LVAV)。采用ELISA法进行MMP-9血清浓度测定。结果 (1)术后1dMMP-9血清浓度开始增高,术后3d达到高峰,至术后4周仍然高于术前,差异有统计学意义(P<0.05)。(2)与术前比较,术后各时间段的左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、LVAV均增大(P<0.05),LVEF下降(P<0.05)。(3)直线相关性显示:术后各时间段MMP-9血清浓度与LVEF均具有相关性,其中术后3dMMP-9血清浓度与LVEF相关性较好(r=0.731,P<0.05)。术后2d后各时间段MMP-9血清浓度与LVAV均具有相关性,其中术后3dMMP-9血清浓度与LVAV相关性较好(r=0.636,P<0.05)。结论 MMP-9在室壁瘤形成过程中对心功能和LVAV有一定的影响作用。Objective To explore the correlation between matrix metalloproteinase-9（MMP-9）and cardiac function,aneurysm volume in the process of left ventricular aneurysm（LVA）formation.Methods Rabbit models of LVA were established in 20 New Zealand rabbits by lighting left anterior descending artery and left circumflex artery.Two-dimensional echocardiography were performed at preoperative and postoperative 1d,2d,3d,1w,2w,3w,4w.The survived animals which had LVA formed were enrolled.Real time three-dimensional echocardiography was performed to obtain left ventricular volume,LVEF and LVA volume,The MMP-9serum concentration was measured by ELISA.Results（1）There were a significant increase in the serum concentration of MMP-9from postoperative 1dand arrived at the peak at postoperative 3d,there were higher than preoperative at postoperative 4w（P〈0.05）.（2）Compared with preoperative,the LVEDV,LVESV and LVAV volume at postoperative showed an increase trend（P〈0.05）,while there was a decreasing trend in LVEF（P〈0.05）.（3）The MMP-9had the better relationship with LVEF at postoperative 1d,2d,3d,1w,2w,3w,4w,in which MMP-9had the tightest relationship with LVEF at postoperative 3d（r=0.731,P〈0.05）.MMP-9had the better relationship with LVA volume at postoperative 2d,3d,1w,2w,3w,4w,in which MMP-9had the tightest relationship with LVA volume at postoperative 3d（r=0.636,P〈0.05）.Conclusion The MMP-9had an effect on cardiac function and LVA volume in the process of LVA formation.
