摘 要:目的初步研究甲状腺肿瘤患者MHC基因频率分布及s MICA表达水平及相互关系。方法提取2个甲状腺肿瘤病例组(44名经术后病理证实为甲状腺癌的患者及87名甲状腺良性肿瘤患者)、1个健康对照组(133名健康体检者)的全血基因组DNA,采用直接测序法做HLA、MICA、MICB高分辨分型;运用Pypop Win32分析各基因位点的基因频率分析;SPSS 17.0分析甲状腺癌患者与MICA/B及HLA等位基因的相关性。运用ELISA法对上述研究对象的血清做s MICA半定量检测。结果 MICB*004∶01在甲状腺癌组、甲状腺良性肿瘤组和健康体检组的频率分别为:1.351%(1/44)、5.769%(6/87)、10.078%(26/133),是免患甲状腺癌患者的保护等位基因[P=0.014,RR(95%CI)=0.122(0.016-0.91)];DRB1*15∶01和DRB1*04∶03为甲状腺良性肿瘤的保护等位基因(P<0.05,RR<1);甲状腺癌患者与甲状腺良性肿瘤及健康体检者血清s MICA水平(ng/m L)分别为:0.233 1±0.244 5 vs 0.1819±0.1947 vs 0.2284±0.2787(P>0.05)。结论 HLA基因与甲状腺肿瘤疾病存在关联,其中MICB*004∶01与患甲状腺癌风险呈负相关。Objective To primarily investigate the genetic immunological background in thyroid tumor patients and asso- ciate thyroid carcinoma with MICA gene expression level in patients sera. Methods Genomic DNA were extracted from two teams with thyroid tumors (44 patients with thyroid carcinoma , 87 with benign thyroid nodules) and one team of healthy controls (133 healthy population through physical examination). The HLA, MICA and MICB alleles were identifie.d:; by SBT. The gene frequency was analyzed by PyPopWin32 software. SPSS 17.0 were used to analyze the association of cancer with MICA/B and HLA gene frequency. ELISA was applied to test the soluble MICA(sMICA) concentration in sera. Results The MICB* 004:01 frequencies in thyroid carcinoma, benign thyroid nodules and healthy population were 1. 351%, 5. 769% and 10. 078%, respectively. MICB* 004:01 was the protective factor of thyroid carcinoma (P = 0. 014, RR [ 95% CI ] = 0. 12210. 016 -0.91 ] ). HLA-DRB1 * 15:01 and DRB1 * 04:03 were the protect I've alleles in benign thyroid nodules ( P 〈 0. 05 ,RR 〈 1 ). The level of sMICA among thyroid carcinoma, benign thyroid nodules and healthy population was (0. 2331±0. 2445) vs (0. 1819±0. 1947) vs (0. 2284±0.2787) ng/ml, respectively. Conduslon HLA system is associated with thyroid tumors, of which MICB*004:01 is negatively correlated with thyroid carcinoma.
