摘 要:目的探讨贵州地区HPA-1-5、15基因多态性与脑梗死发生的相关性。方法采用PCR-SSP方法对本地区随机人群512例脑梗死病例和530例对照进行HPA-1-5、15系统基因分型,通过脑梗死组与对照组基因和基因型比较及Logistic回归分析,研究HPA-1-5、15基因多态性与脑梗死发病的相关性。结果 1 HPA抗原系统多态性的比较:HPA-2差异显著(基因型χ2=58.031 P<0.001、基因χ2=54.899 P<0.001);HPA-1、3、4、5和15系统无显著差异,P值均>0.05。2 HPA-2基因型分层比较:ab、bb和ab+bb均差异显著(χ2=46.775 P<0.001、χ2=9.482 1 P=0.002和χ2=52.084 P<0.001)。3 HPA-2回归分析:以HPA-2aa为参照值1,并进行调整后ab、bb和ab+bb的OR值分别为4.797(95%CI 2.950-7.799)、1 859 232 562.995(95%CI 0-∞)和5.115(95%CI 3.155-8.291)。结论 HPA-2抗原系统基因多态性可能是本地脑梗死发生的遗传易感因素,HPA-2b基因可能是其易感基因,HPA-2ab可能是易感基因型。Objective To investigate the relationship between cerebral infarction and genetic polymorphisms of human platelet antigens 1 to 5 and 15 in Guizhou. Methods 512 cases with cerebral infarction and 530 cases without cerebral in- farction were chosen randomly for comparison. Their genetic polymorphisms of human platelet antigens 1 to 5 and 15 were classified by PCR-SSP. Their relationships were analyzed using logistic regression analysis. Results 1 ) When comparing the polymorphisms of HPA antigen system, the differences among HPA-2 were found to be significant (genotype, χ^2= 58. 031, P 〈0.001 ; gene, χ^2 = 54. 899 ; P 〈 0. 001 ), but those among HPA-1, 3, 4, 5 and 15 systems were not significant( P 〉 0.05 ). 2 ) When comparing HPA-2 genotypes, the differences among ab, bb and ab + bb were found to be significant (χ^2= 46. 775, P 〈 0. 001 : χ^2 = 9. 4821, P = 0. 002 and χ^2 = 52.084. P 〈 0. 001 ). 3) From the HPA-2 recession analysis, if the reference values of HPA-2 aa of OR were 1, then ab,bb and ab + bb values of OR were 4. 797 (95% CI, 2. 950 to 7. 799), 1859232562.995 (95% CI, 0 to infinity) and 5. 115 (95% CI, 3. 155 to 8. 291 ), respectively. Conclusion The HPA-2 genetic polymovphisms may be a factor of genetic susceptibili- ty for the occurrenee of cerebral infarction. HPA-2b gene may be a kind of susceptible gene. HPA-2ab gene may be suscepti- ble to the formation of the genotype.
