乙肝病毒基因型与慢性乙肝患者血清白细胞介素-18及其他细胞因子的关系

摘　　要：目的：探讨慢性乙型肝炎（ CHB）患者B、C基因型血清中白细胞介素-18（ IL-18）和其他细胞因子的关系。方法采用荧光法对医院感染病科收治的100例CHB患者进行B、C基因分型；采用酶联免疫（ ELISA）法检测CHB患者实验组及健康对照组血清细胞因子IL-18、IL-12、干扰素-γ（ IFN-γ）的水平，并比较两组及不同基因型IL-18等细胞因子表达的差异。结果100例CHB患者中，B型70例，C型18例，BC混合型6例，未分型6例；CHB C基因型患者的血清中IL-18、IL-12、IFN-γ因子水平明显高于B基因型患者（P＜0.05）；CHB患者血清中IL-18、IL-12、IFN-γ因子水平均明显高于健康对照组（ P＜0.05），且实验组血清中IL-18、IL-12、IFN-γ的水平与丙氨酸氨基转移酶（ALT）、天门冬氨酸氨基转移酶（AST）、血清胆红素（TBIL）水平呈正相关（P＜0.05）。结论100例CHB患者基因型主要以B型为主；患者血清中IL-18、IL-12、IFN-γ因子水平与肝脏炎症活动有关；在慢性乙型肝炎的炎性活动时，C基因型患者Th1细胞免疫水平强于B基因型患者。Objective To investigate the relationship between genotype B, C of HBV and the levels of IL-18 and other cytokines in chronic hepatitis B（ CHB） patients.Methods One hundred pa-tients with HBV were detected by genotyping fluorescence assay.The serum levels of IL-18, IL-12, IFN-γbetween two groups were detected by ELISA.The differences between IL-18 and other cytokines ex-pressed in two groups and in different genotypes were compared.Results Among the 100 CHB pa-tients, 70 patients were type B, 18 patients were type C, 6 patients were BC mixed type and 6 patients were untyped.The serum levels of IL-18, IL-12, IFN-γin genotype C were significantly higher than those in genotype B（P〈0.05）.The serum levels of IL-18, IL-12, IFN-γin patients with CHB were significantly higher than those in healthy control group（P〈0.05）.The serum levels of IL-18, IL-12, IFN-γin patients with CHB were positively correlated with the levels of ALT, AST, TBIL （ P〈0.05） . Conclusions One hundred CHB patients are mainly genotype B, the serum levels of IL-18, IL-12, IFN-γare correlated with the liver inflammatory activity,Th1 cells immunity levels in genotype C patients are higher than those in genotype B patients when liver inflammation of CHB is proceeding.
