原发性高血压合并糖尿病患者动态血压特点分析

摘　　要：目的探讨原发性高血压合并2型糖尿病患者动态血压的特点。方法选择原发性高血压合并2型糖尿病患者180例作为试验组,单纯原发性高血压患者180例作为对照组,均行动态血压检查,比较两组患者白天、夜间及24小时的血压及其变异性指标。结果试验组夜间平均收缩压、白昼及24 h收缩压标准差明显高于对照组(P<0.05)。两组患者24 h平均收缩压与舒张压、白天收缩压与舒张压。夜间舒张压,24 h舒张压标准差、白天舒张压标准差、夜间收缩压标准差及夜间舒张压标准差比较,差异均无统计学意义(P>0.05)。结论 2型糖尿病可能损害原发性高血压患者收缩压调节功能,动态血压监测可为临床诊治提供有价值的参考。Objective To investigate the characteristics of 24 hour blood pressure change in patients with essential hypertension combined with type 2 diabetes. Methods A total of 180 essential hypertension patients with type 2 diabetes were selected as experimental group while 180 patients with pure primary hypertension were used as control group. All the patients were examined with ambulatory blood pressure. The blood pressures during the daytime and at night as well as in 24 h and variability index were compared between the two groups. Results The nighttime average systolic blood pressure,24 h systolic blood pressure standard deviation and standard deviation of the daytime systolic pressure in the experimental group were significantly higher than that in the control group（ P〈0. 05）. There was no statistical difference in the 24 h mean systolic blood pressure,24 h average diastolic blood pressure,daytime systolic blood pressure,daytime diastolic blood pressure,nighttime diastolic blood pressure,24 h diastolic blood pressure standard deviation,daytime diastolic blood pressure standard deviation,nighttime systolic blood pressure standard deviation and nighttime diastolic blood pressure standard deviation between the two groups（ P〉0. 05）. Conclusion Type 2 diabetes may damage the systolic blood pressure regulative function in essential hypertension patients. The detection of ambulatory blood pressure can provide valuable reference for clinical diagnosis and treatment.
