早期前庭康复训练在治疗慢性主观性头晕的临床应用

摘　　要：目的观察早期前庭康复训练在慢性主观性头晕治疗中的作用。方法对我科自2013年12月～2014年6月收治的48例慢性主观性头晕患者进行对照分组，治疗组给予早期前庭康复训练及认知-行为治疗、抗焦虑药物治疗；对照组给予认知-行为治疗、抗焦虑药物治疗。量化对比观察两组患者经治疗2、4、8周后的疗效。结果两组患者基础DHI、HAMA、HAMD量表数据评价均通过方差齐性检验（P〉0．05）。两组患者经过治疗后眩晕残障及情绪障碍等均明显好转（P〈0．05）。以各量表总分的减分率作为疗效主要统计指标，两组间比较数据显示前庭康复治疗能使患者的头晕症状及焦虑等情绪在早期得到缓解，与对照组比较差异有统计学意义（P〈0．05）。结论前庭康复训练在慢性主观性头晕治疗中具有重要补充作用，越早进行康复训练则疗效越好。Objective To observe the action of earlier vestibule rehabilitation training in treating chronic subjective dizziness. Methods 48 cases of patients with chronic subjective dizziness who were treated in our department between December 2013 and June 2014 were divided into two groups. The treatment group was treated by earlier vestibule reha- bilitation training, cognitive-behavioral therapy and antidepressants The control group was treated by cognitive-behav- ioral therapy and antidepressants. The curative effect of two groups at 2 weeks, 4 weeks, and 8 weeks were respectively observed and contrasted. Results Two groups of patients were evaluated by homogeneity test of variance based on basic DHI, HAMA and HAMD scales（P〉0.05）. Data analysis showed that both of the two groups gained obvious improvement in dizziness handicap and emotional disorder, etc. after treatment （P〈0.05）. With reductive ratio of total scores of each scale as the main statistics index for efficacy, comparison between two groups showed vestibular rehabilitation therapy can relive dizziness handicap and anxiety in an early period, and there were significantly differences compared to the control group （P〈0.05）. Conclusion Vestibule rehabilitation training plays an important supplement role in treatment of chronic subjective dizziness, and the sooner the better.
