摘 要:目的评价胸部创伤检查中DR与CT技术诊断的准确性。方法回顾性分析我院收治的80例胸部创伤患者临床资料,并分别采用DR技术和CT技术进行检查,最终以手术病理结果作为金标准,比较两种技术诊断的符合率和准确性。结果 80例患者中肋骨骨折27例,肺撕裂伤10例,气胸及血气胸9例,纵隔损伤6例,胸腔积液8例,肺不张及肺塌陷4例,皮下气肿4例,复合病变12例,DR诊断符合率77.5%(62/80),其中肺部撕裂漏诊4例,气胸及血气胸漏诊3例,纵隔损伤漏诊2例,胸腔积液漏诊4例,肺不张或者肺塌陷漏诊1例,皮下气肿漏诊2例,复合病变漏诊2例,漏诊率22.5%(18/80)。CT诊断符合率91.25%(73/80),其中气胸及血气胸漏诊1例,纵隔损伤漏诊1例,胸腔积液漏诊3例,肺撕裂伤漏诊1例,复合病变漏诊1例,漏诊率8.75%(7/80),CT诊断的符合率明显高于DR,漏诊率明显低于DR(P<0.05)。结论临床胸部创伤诊断中多层螺旋CT技术诊断的准确性明显优于DR技术,此种诊断技术能够有效为临床急救措施提供切实可行的依据。Objective To discuss the accuracy of DR and CT technology in the diagnosis of chest trauma. Methods A total of 80 cases of chest trauma patients treated in our hospital were chosen, and were given DR and CT examination. Pathologic findings were the gold standard, and compared the coincidence rate and accuracy of the two technologies. Results A total of 80 cases were confirmed by surgery 27 cases of rib fractures, 10 cases of lung laceration, 9 cases of pneumothorax and pneumothorax, 6 cases of mediastinum injury, 8 cases of pleural effusion, 4 cases of atelectasis and lung collapse, 4 cases of subcutaneous emphysema, 12 cases of complex lesions. DR diagnosis coincidence rate was 77.5%(62/80), missed 4 cases of lung tear, missed 3 cases of pneumothorax and pneumothorax, mediastinal injury missed 2 cases, pleural effusion missed 4 cases, atelectasis or lung collapse missed 1 case, subcutaneous emphysema missed 2 cases, 2 cases of complex lesions were missed, missed diagnosis rate was 22.5% (18/80). CT diagnosis coincidence was 91.25%(73/80). Missed 1 case of pneumothorax and pneumothorax, mediastinal injury missed 1 case, pleural effusion missed 3 cases, 1 case of lung laceration, and 1 case of complex lesion, missed diagnosis rate was 8.75%(7/80). CT diagnostic accordance rate was significantly higher than DR, misdiagnosis rate was significantly lower than that of DR (P〈0.05). Conclusion The accuracy of clinical diagnosis of chest trauma of muhislice spiral CT is much better than DR technology, and this diagnostic technique can provide practical basis for clinical and emergency
