摘  要:目的:探讨分析320排螺旋CT脑动静脉双期扫描时相的合理选择以及临床应用价值。方法:回顾性分析2014年10月至2015年1月于我院行全脑动态增强扫描的48例患者的影像资料,进行最大密度投影和容积显示后,经双盲法分别确定每一例脑动脉和静脉显示的最佳时间点,取48例患者最佳的动静脉显示时间点的算数平均值为最佳动静脉期延迟时相。将此时相应用于80例患者的脑血管检查中,采取双盲法对所得脑血管双期影像评分(0~5分),并以合格条件为评分分数≥2分来计算脑血管双期CT检查成功率。结果:最佳动静脉期时相为(20.55±2.38)s和(34.84±2.50)s。动脉期和静脉期检查成像成功率分别为97.5%和96.3%,一次检查合格率为93.8%。结论:320排CT脑血管双期扫描的动静脉期时相为(20.55±2.38)s和(34.84±2.50)s时,一次检查成功率高。可获得满意的纯动脉和静脉图像,为临床提供可靠全面的图像信息。Purpose: To investigate the clinical value and analyze the appropriate moment for double-phase cerebral angiography examination using 320-slice spiral CT.Methods:Totally 48 cases,who underwent whole-brain dynamic CT angiography during the period from 2014 October to 2015 January,were analyzed retrospectively to find the best phase for imaging cerebral arteries and veins. Then the result was applied to 80 cases.The reconstruction techniques including maximum intensity projection(MIP) and volume rendering(VR) were used in all cases. The images quality were graded by 2 readers(5-0 point, 5=excellent) in a double-blind way. When the score was greater than or equal to 2 point, the examination was judged as successful, and the successive rate of this examination was calculated.Results: The optimal time for arterial phase and venous phase was(20.55±2.38)s and(34.84±2.50)s. The successive rate of arterial and venous phase imaging was 97.5% and 96.3%, and the qualified rate of one-off examination was 93.8%. Conclusion: The double phase cerebral angiography imaging by 320-slice CT can obtain the images of the artery and the vein simultaneously. There is a high successive rate of imaging at the phases of(20.55±2.38)s and(34.84±2.50)s. And this imaging method provides reliable and comprehensive information for clinic and has a high application value.
