摘  要:目的:探讨MRI C线位扫描在显示胎儿颅后窝池增宽的临床应用价值。方法:采用Siemens Avanto1.5T超导磁共振扫描仪,对30例B超怀疑颅后窝池增宽的中、晚期孕妇进行MR常规及C线的扫描,分别测量常规横断位及C线位颅后窝池最大宽度值,与B超测量颅后窝池宽度值对照,分别采用配对t检验对最大宽度值进行比较;以B超结果为标准,分别记录各组增宽例数,对各组检出率进行卡方检验。结果:B超测量颅后窝池平均宽度数值为(11.4±2.17)mm,常规横断位测出颅后窝池均值为(13.3±4.69)mm,而C线位测出颅后窝池宽度均值为(11.6±2.62)mm;常规横断位与B超测量值、C线测量值差异均有统计学意义(t=2.386,2.308,P=0.024,0.028<0.05),C线位与B超测量值常差异无统计学意义(t=0.54,P=0.589>0.05)。以B超测量最大宽度值≥10mm来诊断颅后窝池增宽,B超增宽26例(87%),常规横断位增宽21例(70%),C线位增宽25例(83%),三者检出率无显著性差异(P=0.126>0.05)。结论:以B超测量值为标准,C线位对颅后窝池增宽的测量值较常规横断位更具有重要参考价值。Purpose: To evaluate the clinical value of C-line scan in fetal posterior cranial fossa abnormalities. Methods: Thirty pregnant women with suspected fetal posterior cranial fossa abnormalities by ultrasound were undergone MRI scan within one week on a Siemens Avanto1.5 T scanner. All the head of the 30 fetuses were examined using the conventional axial and C line MR scanning. Referring to the result of ultrasound, the maximum diameters of the posterior cranial fossa were recorded and compared by paired t test. The abnormal rate between two groups were compared by Chi square test. Results: Of these 30 cases, 30 fetuses were clearly displayed. The anatomical structures of the thalamus, the cerebellum, posterior cranial fossa were clearly shown on both C line and conventional axial scanning. On the images of the C line, the anatomical structures of the thalamus, the cerebellum, posterior cranial fossa coexisted on the same image; while the conventional axial images could not display the above structure on the same level. The mean maximum diameter of posterior cranial fossa measured by ultrasound, conventional axial and C line was 11.4±2.17 mm, 13.3±4.69 mm and 11.6±2.62 mm respectively. There were significant differences between the results from conventional axial and C line, ultrasound(t= 2.386,2.308, P=0.024,0.028〈0.05), while there was no significant difference between that from ultrasound and C line( t=0.54,P=0.589). Referring to the diagnostic standard(〉10mm), the abnormal number of each group were 26(87%, ultrasound), 25(83%, C line), and 21(70%, conventional axial). There was no significant difference among the three groups(P=0.126, P〉0.05). Conclusion: Referring to the result of ultrasound, the anatomical structure of posterior cranial fossa can be clearly displayed on both the C line and the conventional axial scan, but the accuracy of the C line is higher than that of conventional axial scan.
