肺小气道疾病高分辨率CT影像表现

摘　　要：目的分析肺小气道疾病高分辨率CT(HRCT)的影像特点,探讨HRCT诊断肺小气道疾病的价值。方法选择2013年1月—2015年2月住院治疗,经临床确诊为肺小气道疾病的患者48例为研究对象,所有患者均进行常规CT和高分辨率CT扫描检查,通过分析患者的CT影像特点评价HRCT诊断肺小气道疾病的价值。结果肺小气道疾病常规CT影像表现多样,主要表现为树芽征、边缘不清的小叶中心结节、局限性磨玻璃密度影及气腔样实变、肺密度减低和马赛克灌注;高分辨率CT影像能清晰显示肺小气道病变情况,能客观的反映出肺小气道病变的分布、程度和范围,在高分辨率CT扫描下,肺小气道疾病主要表现为细支气管壁增厚和细支气管腔阻塞二种情况。结论高分辨率CT是诊断肺小气道疾病最好的影像学方法,具有很高的价值,能提供很多用于评价肺小气道形态及功能的信息。Objective To analyze the pulmonary imaging characteristics of high resolution CT（HRCT）on small airway disease,and to explore the value of HRCT in the diagnosis of pulmonary small airway disease.Methods The clinical diagnosis of 48 patients with lung of small airway disease,from January2013 to February 2015 hospitalized in author＇s hospital,were chosen as the research subjects.All patients received conventional CT and high resolution CT scan,and then the value of HRCT was analyzed by the CT imaging features of patients in the diagnosis of pulmonary small airway disease.Results Pulmonary small airway disease conventional CT images showed diversity,main showed was unclear,edge tree-in-bud lobules centre nodules,limitations of ground glass density and consolidation of the sample chamber,lung density and Mosaic perfusion;High resolution CT images clearly showed small airway disease in lung,could objectively reflect the distribution,degree and scope of pulmonary small airway lesions,under the high resolution CT scan,lung small airway disease mainly characterized by thin bronchial wall thickening and bronchioles cavity block two cases.Conclusion High resolution computed tomography（CT）is by far the most good small airway disease diagnosis lung imaging methods,has the very high value,can provide a lot of information to evaluate lung small airway morphology and function.
