摘　　要：【目的】探讨AO重建钢板联合Herbert螺钉治疗肱骨髁间粉碎性骨折的临床效果。【方法】2008年5月至2013年8月采用A0重建钢板联合Herbert螺钉治疗26例肱骨髁间粉碎性骨折患者，根据Muller的AO分型标准均为C3型骨折。所有患者均取后正中切口，采用尺骨鹰嘴截骨入路。【结果】所有患者均获得14（12～18）个月随访。术后12个月，肘关节功能按改良Aitken-Rorabeck评分系统进行评定。术后疗效为优15例、良9例、可2例，优良率为92．30％（24／26）。【结论】采用AO重建钢板联合Herbert螺钉治疗肱骨髁间粉碎性骨折，固定牢固，患者术后可以早期功能锻炼，其疗效优于其它内固定方法，可作为目前手术治疗肱骨髁间粉碎性骨折首选方法。[Objective]To evaluate the curative effect of AO reconstruction plate combined with Herbert screws for intercondylar comminuted fracture of humeral. [Methods]From May 2008 to August 2013 , 26 cases of humeral intercondylar fractures were treated with AO reconstruction plating and Herbert screw. According to the Muller AO classification , 26 cases were identified as type C3. All patients were treated by posterior midline incision with olecranon osteotomy approach. [Results] All of the cases were followed up ranged from 12 to 18 months with an average of 14 months. 12 months after operation, the elbow joint function was assessed by the modified Aitken-Rorabeck scoring system. The results showed ：Excellent in 15 cases, Good in 9 cases and Ordinary in 2 cases, the excellent and good rate was 92.30% （24/26）. [Conclusion]AO reconstruction plats and Herbert screw for intercondylar comminuted fracture of humerals showed a good performance of firm fixation. After operation the patients can carry out functional training in early stage , and the effect is better than that of other internal fixation. It can be used as the first choice for the treatment of intercondylar comminuted fracture of humeral as the internal fixation materials.
