摘 要:目的通过比较不同呼气末正压通气(positive end expiratory pressure,PEEP)治疗手足口病并神经源性肺水肿的效果以及对循环呼吸的影响,探讨更具优越性的通气方式及可能的作用机制。方法选择2011年1月—2014年6月收治的80例手足口病并神经源性肺水肿(肺出血)患儿作为研究对象,患儿均经气管插管、机械通气加用PEEP,随机分为对照组及观察组各40例。对照组PEEP设置为3-8 cm H2O(1 cm H2O=0.098 k Pa),观察组PEEP设置为8-20 cm H2O。观察机械通气后24 h患儿的生命体征、血气分析、血流动力学指标及7 d后格拉斯哥昏迷评分(Glasgow coma score,GCS)、急性生理与慢性健康评分Ⅱ(acute physiology and chronic health evaluationⅡ,APACHEⅡ)、胃肠功能评分得分情况。同时观察胸片改善时间、机械通气时间及住院时间。计量资料采用t检验,P〈0.05为差异有统计学意义。结果观察组治疗24 h后,体温(temperature,T)、呼吸频率(respiratory rate,RR)、心率(heart rate,HR)分别为(38.5±0.4)℃、(28.5±3.2)、(118.0±8.5)次/min,与对照组的(38.2±0.5)℃、(36.4±4.5)、(132.0±7.0)次/min比较差异均有统计学意义(均P〈0.05)。观察组Pa O2、Pa CO2、LAC、氧合指数分别为(84.3±9.8)、(38.0±5.8)mm Hg(1 mm Hg=0.133 k Pa)、(2.58±1.31)mmol/L、(189.2±40.2)mm Hg与对照组的(65.5±13.5)、(43.7±6.2)mm Hg、(6.42±1.38)mmol/L、(135.0±22.3)mm Hg比较差异均有统计学意义(均P〈0.05)。观察组平均动脉压(mean arterial pressure,MAP)、中心静脉压(central venous pressure,CAP)、血管外肺水(extravascular lung water,EVLW)分别为(60.5±11.6)、(0.88±0.34)mm Hg、(7.53±1.21)ml/kg与对照组的(82.3±13.1)、(1.02±0.27)mm Hg、(15.67±3.11)ml/kg比较差异均有统计学意义(均P〈0.05)。观察组GCS、APACHEⅡ得分分别为(11.6±2.5)、(15.4±3.7Objective Investigate the effect of different positive end expiratory pressure(PEEP) for the treatment of hand-footmouth disease, companied by neurogenic pulmonary edema. Methods 80 cases with hand-foot-mouth disease companied by neurogenic pulmonary edema from January 2011 to June 2014 were enrolled and randomly divided into control group and observation group(n=40). The patients in two groups were given mechanical ventilation, the control group PEEP set to 3-8 cm H2O( 1 cm H2O=0.098 k Pa), the observation group PEEP set to 8-20 cm H2 O. The data about vital signs, blood-gas analysis, blood biochemistry, hemodynamic condition as well as Glasgow coma score( GCS), acute physiology and chronic health evaluationⅡscore( APACHEⅡ) and gastrointestinal function score after 24 h were collected and analyzed. Meanwhile, time of improved chest X-rays, duration of mechanical ventilation and length of hospital stay were also detected. Measurement data was processed by t test, P〈0.05 was considered statistically significant. Results Temperature(T)、respiratory rate(RR) and heart rate(HR) of observation groups were(38.5 ±0.4)℃,(28.5 ±3.2),(118.0 ±8.5) beats/min, in the control group were(38.2 ±0.5)℃,(36.4 ±4.5),(132.0±7.0) beats/min,the differences were statistically significant(all P〈0.05).
