糖皮质激素联合玻璃酸钠关节腔内注射治疗膝关节骨性关节炎的临床研究

摘　　要：目的:探究糖皮质激素联合玻璃酸钠关节腔内注射治疗膝关节骨性关节炎的临床疗效。方法:选取2012年6月至2014年6月期间我院接收的120例患有膝关节骨性关节炎的患者,并按照随机数字数表法分为观察组和对照组,每组60例,均给予膝关节腔内注射2.5 m L玻璃酸钠,在治疗第一周给予观察组患者膝关节腔内注射1 m L复方倍他米松,对照组给予相同剂量的注射用水;分别对治疗前、治疗后1周、2周、1个月、3个月、5个月时患者膝关节Lequence评分,并对比两组治疗效果。结果:治疗后,两组膝关节Lequence评分均显著低于治疗前,且观察组降低更明显,差异均有统计学意义(P<0.05);观察组患者治疗后1周、2周、1个月、3个月、5个月总有效率均高于对照组,且差异具有统计学意义(P<0.05)。结论:糖皮质激素联合玻璃酸钠关节腔内注射治疗膝关节骨性关节炎具有良好的临床疗效,为临床治疗提供了新的思路。Objective： To explore the clinical curative effect of the glucocorticoid combined with intra-articular injection of sodium hyaluronate in treatment of knee osteoarthritis. Methods： A total of 120 patients with knee osteoarthritis, who were treated in Mianyang Central Hospital of Sichuan Province from June 2012 to June 2014, were randomly divided into observation group（n=60） and control group（n=60）. All the patients were given the injection of 2.5 m L sodium hyaluronate into the knee joint cavity once a week for 5weeks. In the first week of treatment, the observation group was injected 1 m L compound betamethasone into the knee joint cavity, while the control group, the same dose of injection water into the knee joint cavity. The knee joint Lequence scores and total effective rates of the two groups were compared. Results： After treatment, Lequence scores of the knee joint of the two groups were significantly lower than before treatment, and the observation group decreased more significantly, the difference was statistically significant（P〈0.05）. The total effective rates of 1 week, 2 weeks, 1 month, 3 months, 5 months after treatment in the observation group were higher than those in the control group, the differences were statistically significant（P〈0.05）. Conclusion： The glucocorticoid combined with intra-articular injection of sodium hyaluronate in the treatment of knee osteoarthritis has a good clinical effect,which provides a new thinking for clinical treatment.
