入院血糖水平对初发非糖尿病急性心肌梗死患者院内死亡的影响

摘　　要：目的探讨入院时血糖水平与初发非糖尿病急性心肌梗死患者院内死亡的相关性。方法观察性分析2008年1月至2014年6月在沭阳县人民医院住院期间初次诊断为急性心肌梗死的非糖尿病患者480例。以入院血糖不同水平将已知非糖尿病的患者分组：入院血糖水平〈6.1 mmol/L组（137例）,6.1-7.7 mmol/L组（146例）,7.8-11.0 mmol/L组（129例）,11.1-13.0 mmol/L组（31例）和〉13.0 mmol/l组（37例）,后三组定义为入院高血糖组。分析各组患者住院期间的病死率。结果随血糖水平升高,非糖尿病患者病死率呈逐渐增加趋势（血糖〈6.1 mmol/L组0.7%,6.1-7.7 mmol/L组6.2%,7.8-11.0 mmol/L组10.9%,11.1-13.0 mmol/L组19.4%,〉13.0 mmol/L组27.0%）。多变量Logistic回归分析显示,在非糖尿病患者中,随血糖升高死亡危险逐步增加（血糖6.1-7.7 mmol/L组：OR=8.95,P=0.43;血糖7.8-11.0 mmol/L组：OR=28.33,P〈0.01;血糖11.1-13.0 mmol/L组：OR=27.17,P〈0.01;血糖〉13.0 mmol/L组：OR=36.95,P〈0.01）。结论既往无糖尿病史的初发急性心肌梗死患者入院血糖水平升高也很常见,且是与院内死亡密切相关的危险因素。Objective To evaluate the impact of admission blood glucose level on hospital mortality in intial non-diabete acute myocardial infarction. Methods Four hundred and eighty patients diagnosed as acute myocardial infarction from 2010 to2014 in Shuyang People＇s Hospital were selected. The patients without diabetes were divided into different groups by the admission blood glucose level： 6. 1 mmol / L（ n = 137）,6. 1- 7. 7 mmol / L（ n = 146）,7. 8- 11. 0 mmol / L（ n = 129）,11. 1- 13. 0mmol / L（ n = 31）, 〉13. 0 mmol / L（ n = 37）,the last three groups were defined the hyperglycemia group. The fatality rate among the five groups were analyzed. Results The fatality rate of nondiabetic patients was gradually increasing with the increasement of glucose levels. Multivariate logistic regression analysis showed that the incidence of hospital mortality gradually increased in acute myocardial infarction patients without known diabetes with the increasement of glucose levels. Conclusion The glucose levels of acute myocardial infarction patients without diabetes increasing are common,and it is closely correlated with higher hospital mortality.
